摘要
目的探讨亚甲基四氢叶酸还原酶(MTHFR)rs1801133和rs1801131基因多态性与银川回族人群高血压和血浆同型半胱氨酸(Hcy)水平的相关性。方法选择2016年1月至2017年12月,宁夏医科大学总医院被诊断为原发性高血压的回族患者770例,回族血压正常对照1299人作为研究对象,平均年龄45.73岁,收集研究对象基本资料,检测Hcy、血脂、空腹血糖、尿酸等指标。应用基质辅助激光解吸附电离飞行时间质谱分析技术(MassArray system)对样本进行基因分型。结果高血压组MTHFRrs1801133的CC、CT、TT 3种基因型频率和C、T两种等位基因频率与血压正常组比较,差异有统计学意义(P<0.05)。高血压组MTHFRrs1801131的AA、AC、CC 3种基因型频率和A、C两种等位基因频率与血压正常组比较,差异有统计学差异(P<0.05)。MTHFRrs1801133不同基因型间血压、Hcy水平差异有统计学意义(P<0.01),TT和CT基因型Hcy和血压水平高于CC基因型[收缩压:(131.0±18.4),(130.1±19.3)比(127.3±18.0)mm Hg,舒张压:(82.4±12.9),(81.2±13.2)比(79.8±12.1)mm Hg,Hcy:(16.47±5.39),(13.22±3.92)比(7.21±3.09)μmol/L,均P<0.01]。rs1801131各基因型间Hcy水平差异有统计学意义(P<0.01),血压差异无统计学意义(P>0.05)。Logistic回归分析显示,在校正了性别、年龄、体质量指数(BMI)、尿酸及Hcy后MTHFRrs1801133不同基因型与高血压有相关性(TT比CC基因型:OR=3.709,95%CI2.534~5.218,P<0.001),但是rs1801131不同基因型与血压无相关性(P>0.05)。结论 MTHFRrs1801133基因多态性与银川回族人群高血压发病有关,rs1801131与高血压发病无关。
Objective To examine the association of methylenetetrahydrofolate reductase (MTHFR) rs1801133 and rs1801131 gene polymorphisms with hypertension and plasma homocysteine (Hcy) levels in a Ningxia Yinchuan Hui ethnic population. Methods Between January 2016 and December 2017, 770 Hui ethnic essential hypertensives and 1299 normotensives from General Hospital of Ningxia Medical University were enrolled. The average age was 45.73 years old. The basic information was collected and the levels of plasma Hcy, blood lipids, fasting blood glucose (FBG) and uric acid (UA) were measured. MassArray system was used for gene polymorphism analysis. Results The frequencies of CC, CT and TT genotypes or C and T allele frequencies of MTHFR rs1801133 in hypertensives group were significantly different from those in normotensives group (P〈0.05). The frequencies of AA, AC and CC genotypes or A and C allele frequencies of MTHFR rs1801131 were also significant different between above groups (P〈0.05). The blood pressure and Hcy levels were significantly different among different genotypes of MTHFR rs1801133 (P〈0.01) with both higher in TT and CT genotype group than CC group [systolic blood pressure: (131.0± 18.4), (130.1± 19.3) vs (127.3±18.0) mm Hg, diastolic blood pressure: (82.4± 12.9), (81.2±13.2) vs (79.8±12.1) mm Hg, Hcy: (16. 47±5.39), (13.22±3.92) vs (7.21±3.09) μmol/L, allP〈 0.01]. Among different rs1801131 genotype groups there were significant differences in Hcy levels (P〈0.01),while no statistical difference in blood pressure (P〉0.05). After adjusted for sex, age, body mass index (BMI), uric acid and Hcy, logistic regression analysis showed that genotypes of MTHFR rs1801133 were associated with hypertension (TT vs CC: OR=3. 709, 95% CI 2. 53μ5. 218, P〈0. 001), however genotypes of rs1801131 were not associated with hypertension.
