B3型胸腺瘤和胸腺癌中MGMT蛋白表达缺失的意义及预后分析 被引量:2

摘要 目的探讨B3型胸腺瘤和胸腺癌的O6-甲基鸟嘌呤-DNA甲基转移酶(MGMT)蛋白表达缺失的意义以及对预后的影响。方法选取河北医科大学附属第四医院2008年1月—2015年1月行根治性手术切除的胸腺上皮肿瘤标本蜡块60例,按照WHO组织学分型分为B3型胸腺瘤组(20例)、胸腺癌组(28例)、瘤旁组织组(12例);按照Masaoka临床分期分为Ⅰ/Ⅱ组和Ⅲ/Ⅳ组。术后随访截止日期2018年4月。比较B3型胸腺瘤和胸腺癌中MGMT蛋白表达缺失的情况。结果胸腺癌组MGMT蛋白表达缺失高于B3型胸腺瘤组和瘤旁组织组(P <0. 05),B3型胸腺瘤组MGMT蛋白缺失高于瘤旁组织组(P <0. 05)。Ⅲ/Ⅳ期B3型胸腺瘤和胸腺癌发生蛋白表达缺失的比例高于Ⅰ/Ⅱ期(P <0. 05)。MGMT蛋白表达缺失病例5年生存率低于表达正常病例(P <0. 05)。Ⅲ/Ⅳ5年生存率低于Ⅰ/Ⅱ(P <0. 05)。结论 MGMT蛋白表达缺失提示B3型胸腺瘤和胸腺癌恶性程度高,预后差。 Objective To investigate significance of deficiency of O6-methylguanine-DNA methltransferase (MGMT) protein expression in type B3 thymoma and thymic carcinoma and its effect on prognosis. Methods A total of 60 paraffin block samples of thymic epithelial tumor undergoing radical resection between January 2008 and January 2015 were divided into type B3 thymoma group ( n =20), thymic carcinoma group ( n =28) and peripheral tissues of tumor group ( n =12) according to WHO histological typing, and the samples were divided into I/II group and III/IV group according to Masaoka clinical stage. The follow-up deadline was in April 2018. Deficiency condition of MGMT protein expressions in type B3 thymoma and thymic carcinoma was compared. Results Incidence rate of MGMT protein expressions deficiency in thymic carcinoma group was significantly higher than those in type B3 thymoma and peripheral tissues of tumor groups ( P 〈0.05), and the rate in type B3 thymoma group was significantly higher than that in peripheral tissues of tumor group ( P 〈0.05). Ratios of patients having deficiency of MGMT protein expression in III/IV stage were significantly higher than those in Ⅰ/Ⅱ stage B3 thymoma and thymic carcinoma ( P 〈0.05). The 5-years survival rate of patients with MGMT protein expression deficiency was significantly lower than that of patients with normal expression ( P 〈0.05). The 5-years survival rate in III/IV group was significantly lower than that in I/II group ( P 〈0.05). Conclusion Deficiency of MGMT protein expression indicates that B3 thymoma and thymic carcinoma are more malignancy with poor prognosis.
