二维超声联合动态三维诊断不同孕期胎儿畸形临床分析

摘要 目的探讨二维超声联合动态三维诊断不同孕期胎儿畸形临床效果。方法方便选择2016年2月—2018年3月该院妇产科收治的308例产妇,依据孕周不同,将其分为早期组(22~26周)与晚期组(27~40周),每组分别为154例,分别对所有产妇进行二维超声检测,之后对存在疑似情况的产妇进行动态三维超声诊断,比较二维检测方式与联合检测方式对不同孕期胎儿畸形的临床诊断情况,并统计二维超声与联合动态三维超声关于胎儿畸形类型的检出率结果,同时比较两种检测方式对胎儿体表畸形的检测结果。结果二维超声在不同孕期的胎儿畸形确诊率分别为64.52%与77.59%,明显低于联合检测方式的87.10%与91.38%(X^2=4.309,4.209,P<0.05),并且二维超声的胎儿畸形检出率为59.55%,明显低于联合检测方式的95.51%(X^2=33.020,P<0.01),同时二维超声关于胎儿体表畸形的检测准确率为57.41%,明显低于联合诊断方式的94.44%(X^2=20.263,P<0.01)。结论二维超声联合动态三维诊断不同孕期胎儿畸形的临床诊断具有显著效果,不仅能够有效的进行确诊,同时还能够有效检测出胎儿的畸形类型,具有较高的临床推广价值。 Objective To investigate the clinical effects of two-dimensional ultrasound combined with dynamic three-dimensional diagnosis of fetal malformation in different stages of pregnancy. Methods A total of 308 women who underwent obstetrics and gynaecology in the hospital from February 2016 to March 2018 were convenient enrolled. According to different gestational weeks, they were divided into early group(22-26 weeks) and late group(27-40 weeks). There were 154 cases in each group, and two-dimensional ultrasound examination was performed on all the women. Then, the dynamic threedimensional ultrasound diagnosis was performed on the pregnant women with suspected conditions, and the clinical diagnosis of fetal malformation in different pregnancy was compared by two-dimensional detection method and combined detection method. The results of the two-dimensional ultrasound and the combined dynamic three-dimensional ultrasound on the detection rate of fetal malformation were compared, and the detection results of the two detection methods on the fetal surface malformation were compared. Results The diagnosis rate of fetal malformation in two-dimensional ultrasound was 64.52%and 77.59%, respectively, which was significantly lower than 87.10% and 91.38% of the combined detection method(X^2=4.309 4.209, P〈0.05), and two-dimensional ultrasound detection rate of fetal malformation was 59.55%, which was significantly lower than 95.51% of the combined detection method(X^2=33.020, P〈0.01). The accuracy of two-dimensional ultrasound on the detection of fetal surface malformation was 57.41%, which was significantly lower than the joint diagnosis of94.44% of the mode(X^2=20.263, P〈0.01). Conclusion Two-dimensional ultrasound combined with dynamic three-dimensional diagnosis of fetal malformation in different stages of pregnancy has a significant effect, not only can effectively diagnose, but also can effectively detect the type of fetal malformation, with a high clinical value.
