摘要
目的探讨183例绝经后妇女不规则阴道出血的病因。方法回顾性分析该院2016年1月—2017年12月期间收治的183例绝经后不规则出血病例,分析其发病原因及临床表现。结果 183例患者中子宫内膜癌9例,宫颈癌8例,复杂性子宫内膜增生7例,简单性子宫内膜增生30例,宫颈病变(CINII6例,CINIII3例),其余120例为炎症导致,其中43例为慢性宫颈炎、48例为老年性阴道炎、29例为子宫内膜炎。恶性肿瘤患者年龄、绝经年限、出血时间与癌前病变、炎症患者比较,差异有统计学意义(P<0.05)。结论炎症、癌变、子宫内膜增生均会导致绝经后妇女阴道不规则出血,患者绝经年限越长、出血距离绝经的时间越长患者恶性肿瘤的患病几率越大。因此针对年龄较大、绝经年限越长、不规则出血时间越长的患者应嘱其尽早检查,以明确病因进行治疗。
Objective To investigate the etiology of irregular vaginal bleeding in 183 postmenopausal women. Methods A retrospective study of 183 cases of postmenopausal irregular bleeding in the hospital from January 2016 to December 2017 was conducted to analyze the causes and clinical manifestations. Results Of the 183 patients, 9 were endometrial cancer, 8 were cervical cancer, 7 were complicated endometrial hyperplasia, 30 were simple endometrial hyperplasia, 6 were cervical lesions（6 in CINII, 3 in CINIII）, and the remaining 120 were inflamed. As a result, 43 cases were chronic cervicitis, 48 cases were senile vaginitis, and 29 cases were endometritis. The age, menopausal age, and bleeding time of patients with malignant tumors were significantly different from those of precancerous lesions and inflammatory patients（P〈0.05）. Conclusion Inflammation, carcinogenesis, and endometrial hyperplasia can cause irregular vaginal bleeding in postmenopausal women. The longer the menopausal period and the longer the bleeding distance, the longer the risk of malignant tumors.Therefore, patients with older age, longer menopause, and longer irregular bleeding should be examined as soon as possible to determine the cause of the disease.
