摘要
[目的]举要介绍近代浙东名医范文虎脾胃病证治经验。[方法]通过梳理范文虎诊治脾胃病医案及他人相关学术研究论文,整理其诊治脾胃病方面的内容,从范氏治疗呕吐、吐血和黄疸三种病症着手,挖掘整理其对此三种病证的辨证论治、遣方用药的经验。[结果]范氏辨治呕吐多从脾、胃、肝出发,辨证以脾胃虚弱、肝胃不和为多见,治用温中运脾、疏肝和胃为主;辨治吐血,将其病根分虚、瘀,治疗以温补中焦和活血化瘀为主,佐以滋阴或清热;辨治黄疸,按病程将其分为黄疸初起与黄疸日久,初起清利退黄,日久峻下制胜。治疗时对经方、验方亦是灵活运用,变化多端,恰到好处。[结论]范文虎诊治呕吐、吐血、黄疸经验独到,辨证确的,治疗多以经方为主,亦有时方加减,用药至多八九味,药简而力宏,值得中医临床学习借鉴。
[Objective]To introduce FAN Wenhu＇s experience in treating spleen and stomach diseases by studying his medical records. [Methods] By combining the medical cases of spleen and stomach disease and other related academic research papers, sorting out the contents of the diagnosis and treatment of spleen and stomach diseases by FAN Wenhu, starting from the three diseases such as vomiting, hematemesis and jaundice, the author excavated the experience of syndrome differentiation and treatment of the three kinds of syndromes. [Results] FAN＇s treatment of vomiting was mostly based on spleen, stomach and liver. The pathogenesis was deficiency of spleen and stomach or disharmony between liver and stomach. The therapeutic methods were warming spleen Yang and activating spleen Qi or dispersing stagnated liver Qi for regulating stomach. Pathogen of hematemesis was classified into Yang deficiency and blood stasis. The therapeutic methods were mainly warming and nourishing middle-Jiao or promoting blood circulation for removing blood stasis. Nourishing Yin or clearing away heat was used as an adjunctive therapy. According to the course of disease, the jaundice was classified into early stage jaundice and long-term jaundice. At the early stage FAN used the method of clearing away heat and diuresis. While for the long-term jaundice, he used the method of drastic purgation. His application of classical prescriptions was flexible and changeable. [Conclusion] FAN Wenhu＇s experience in diagnosis and treatment of vomiting, hematemesis and jaundice was unique. His diagnosis and treatment were precise and worth to be studied.
