摘要
目的探讨无明显糖尿病视网膜病变(NDR)和轻度非增殖性糖尿病视网膜病变(NPDR)患者黄斑区视网膜厚度、视神经纤维层厚度的变化规律,以发现更早期糖尿病视网膜病变(DR)。方法随机选取126例DR患者作为实验组(轻度NPDR患者76例和NDR患者50例)。选取30例健康人作为对照组。研究对象均行光学相干断层扫描检查(OCT),测量黄斑区6 mm视网膜厚度(RT)和神经纤维层厚度(RNFL)及黄斑区总的容积(TV)。结果 NDR组、NPDR组与对照组中央区、内环区RT比较,P均<0.05;NDR组与NPDR组中央区、内环区RT比较,P均>0.05;三组外环区RT比较,P均>0.05。NDR组与对照组RNFL比较,P均>0.05;NPDR组与NDR组、对照组RNFL比较,P均<0.05。三组TV比较,P均>0.05。结论 OCT能定量观察糖尿病患者黄斑区RT厚度,能够早期发现黄斑水肿,对DR诊断有重要的临床价值。
Objective To investigate the changes of macular retinal thickness and retinal fiber layer thickness in nonproliferative diabetic retinopathy （NPDR） and non-obvious diabetic retinopathy mild （NDR） to discover early diabetic retinopathy（DR）. Methods 126 cases of DR patients from Heze Municipal hospital were randomly selected, 76 cases in NPDR group and 50 cases in NDR group. 30 healthy cases were randomly selected as the control group. Reti- nal thickness （RT） and never fiber layer thickness （RNFL） were measured by optical coherence tomography （OCT） to compare the differences among the three groups. Results The RT in central region and inner ring region of NPDR group and NDR group were compared with that in the control group with P〈0.05; The RT in central region and inner ring region of NPDR group were compared with that in NDR group with P〉0.05; the RT in the outer ring region were compared among the three group with P〉0.05. The RNFL of NDR group was compared with that in the control group with P〉0.05. The RNFL of the three groups were compared with P〈0.05. The TV of the three groups were compared with P〉0.05. Conclusions OCT could assesses the macular retinal thickness and find out the macular oedema of early stage, which have great clinical value in the diagnosis of diabetes retinopathy.
