摘要
目的探讨经皮骶髂关节空心螺钉内固定联合外固定架治疗C型骨盆骨折的临床疗效。方法将86例C型骨盆骨折患者随机分为研究组(经皮骶髂关节空心螺钉内固定联合外固定架)与对照组(常规开放内固定术),各43例。比较两组的治疗效果。结果研究组的手术时间、切口长度、术中出血量、术中透视次数、住院时间、愈合效果、并发症总发生率、Matta评分和Majeed功能评分优良率均优于对照组(P<0.05);术后3 d,两组IL-1β水平均降低,IL-10水平均升高,且研究组优于对照组(P<0.05)。结论采用经皮骶髂关节空心螺钉内固定联合外固定架治疗C型骨盆骨折效果显著且安全性高,值得推广。
Objective To explore the clinical efficacy of percutaneous sacroiliac joint hollow screw internal fixationcombined with external fixation in the treatment of C-type pelvic fractures. Methods A total of 86 patients with C-type pelvicfracture were randomly divided into study group （percutaneous sacroiliac joint hollow screw internal fixation combined withexternal fixation） and control group （conventional open intemal fixation）, with 43 cases in each group. The clinical efficaciesof the two groups were compared. Results The operation time, incision length, amount of intraoperative blood loss,intraoperative perspective frequencies, hospital stays, healing effect, total incidence of complications, the excellent and goodrates of Mata score and Majeed functional score in the study group were significantly better than those of the control group（P〈0.05）. At 3 days after surgery, IL-1β level decreased and IL-10 level increased in both groups, and those in the studygroup were superior to the control group （P〈0.05）. Conclusion Percutaneous sacroiliac joint hollow screw internal fixationcombined with external fixation in the treatment of C-type pelvic fractures is effective and safe, and it is worthy of promotion.
