合肥市孕产妇与新生儿麻疹抗体水平及其影响因素研究 被引量:2

Measles antibody levels among pregnant women and their infants in Hefei city

摘要 目的分析合肥市孕产妇和新生儿麻疹抗体水平及其影响因素,以及新生儿麻疹抗体水平随时间的动态变化。方法在全市随机抽取9家产科医院,采集2014-2015年分娩的孕产妇及其新生儿出生时、6-7月龄的血标本,采用酶联免疫吸附试验检测血清麻疹IgG抗体。结果孕产妇麻疹抗体阳性率为77.0%(455/591),保护率为33.5%(198/591),几何平均浓度(GMC)为461.4mIU/mL;3项指标均以≥28岁孕产妇显著高于〈28岁孕产妇、本市户籍显著高于外市户籍。新生儿出生时麻疹抗体阳性率为87.0%(514/591),保护率为44.7%(264/591),GMC为660.6mIU/mL;3项指标均显著高于孕产妇。母亲与新生儿的麻疹抗体对数浓度呈正相关(r=0.75,P〈0.001)。随访至6-7月龄的婴儿麻疹抗体阳性率为18.6%(51/274),保护率为9.5%(26/274),GMC为35.8mIU/mL。结论孕产妇与新生儿的麻疹抗体水平呈正相关,新生儿麻疹抗体水平在6月龄以后几乎无保护作用。 Objective To assess measles antibody levels in pregnant women and their infants and determine dynamic changes of infant measles antibody levels by infant age. Methods We randomly selected 9 maternity hospitals of Hefei city during 2014-2015 and collected blood specimens from pregnant women, and from their newborn infants at birth and 6-7 months of age for measuring measles IgG antibody using the ELISA technique. Results Among the pregnant women enrolled in the study, the positivity rate, protection rate, and geometric mean concentration of measles antibodies were 77.0%(455/591), 33.5%(198/591), and 461.4 mIU/mL, respectively; values for these variables were significantly higher in ≥28-year-olds than in 28-year-olds and among local-born compared with migrant mothers. Among newborns, the 3 indicators at birth were 87.0%(514/591), 44.7%(264/591), and 660.6 mIU/mL, respectively; each value among the newborn infants was significantly higher than the corresponding value among mothers. There was a positive correlation between maternal and neonatal measles antibody(logarithmic concentration)(r=0.75,P〈0.001). At 6-7 months of age, the 3 indicators among infants were 18.6%(51/274), 9.5%(26/274), and 35.8 mIU/mL. Conclusions Maternal and neonatal measles antibody levels were positively correlated; neonatal measles antibody levels were almost not protective after 6 months of age.
