摘要
目的比较不同心电图(ECG)亚型的完全性左束支传导阻滞(CLBBB)患者的右室功能,评价不同亚型ECG的CLBBB患者右室功能的差异。方法同期收集CLBBB患者58例和26例健康体检者(对照组)。根据ECG的I、V5或V6导联T波与QRS主波方向是否一致,将CLBBB患者分为同向型(cLBBB)(n=30)组和异向型(dLBBB)组(n=28)。对三组均应用常规超声心动图及二维斑点追踪技术评估其右室功能。结果 (1)与对照组比较,cLBBB组及dLBBB组的三尖瓣环收缩期位移、右室整体纵向应变及游离壁整体纵向应变均减低,且dLBBB组低于cLBBB组(P<0.05)。(2)dLBBB组三尖瓣E/e′大于对照组(P<0.05),而cLBBB与对照组无明显差异(P>0.05)。(3)cLBBB组及dLBBB组右室心肌做功指数增加,且dLBBB组高于cLBBB组(P<0.05)。结论 CLBBB患者的右室收缩和整体功能均减低,且异向型较同向型进一步减低。
Objective To evaluate and compare the right ventricular function in complete left bundle branch block (CLBBB) patients with different electrocardiogram subtypes using two-dimensional speckle tracking echocardio- graphy (STE). Methods Fifty-eight patients with CLBBB were selected using standard 12-lead electrocardio- gram, and they were allocated to two subgroups: cLBBB (concordant) group(n = 30) and dLBBB (discordant) group(n=28) according to whether the T wave in lead I, V5 or V6 was concordant with QRS wave or not on elec- trocardiogram. And 26 healthy individuals were included as control group during the study period. They all under- went echocardiography and STE examination. Results ①Tricuspid annular plane systolic excursion, right ven- tricular systolic global longitudinal strain and lateral wall systolic global longitudinal strain decreased in cLBBB and dLBBB patients compared to controls, and more significantly decreased in dLBBB patients (P〈0.05). ②Tricuspid E/e' increased in dLBBB patients (P〈0.05). while there were no significant difference between cLBBB patients and controls (P〉0.05).③Right ventricutar index of myocardial performance increased in cLBBB and dLBBB patients, and more significantly increased in dLBBB patients (P〈0.05). Conclusion The right ventricular function de- creased in patients of CLBBB, and in CLBBB patients, dLBBB is associated with a worse right ventricular systolic, diastolic and total performance than that in cLBBB patients.
