冠心病患者动脉粥样硬化性颈动脉狭窄筛查及危险因素分析 被引量:3

摘要 目的分析冠心病患者动脉粥样硬化性颈动脉狭窄筛查及危险因素。方法将2017年4月—2018年4月于该院接受治疗的90例冠心病患者作为调查对象,根据患者是否存在动脉粥样硬化性颈动脉狭窄症状划分为CAS组与非CAS组(n=45)。对比两组患者的临床资料,明确冠心病患者动脉粥样硬化性颈动脉狭窄的危险因素。结果与非CAS组相比,CAS组患者的年龄均数为(60.04±3.41)岁,存在吸烟史、脑卒中史的患者数量分别为22例、8例组间差异有统计学意义(P<0.05)。同时动脉粥样硬化性颈动脉狭窄与粥样板块发生率之间具有密切关联,CAS组患者的CCA粥样斑块发生率为64.44%,ICA粥样斑块发生率为17.78%;非CAS组患者的CCA粥样斑块发生率为6.67%,ICA粥样斑块发生率为2.22%,差异有统计学意义(χ~2=8.91、5.87,P<0.05)。结论冠心病患者动脉粥样硬化性颈动脉狭窄的发生,与其年龄特点、疾病史以及生活习惯等具有密切关联,且动脉粥样硬化性颈动脉狭窄患者粥样斑块发生率较高,需要提升临床筛查与危险因素分析的重视程度。 Objective To analyze the atherosclerotic carotid stenosis screening and risk factors in patients with coronary heart disease. Methods 90 patients with coronary heart disease who were treated in the hospital from April 2017 to April 2018 were enrolled in the study. According to the presence or absence of atherosclerotic carotid stenosis, they were divided into CAS group and non-CAS group (n= 45). The clinical data of the two groups of patients were compared to determine the risk factors for atherosclerotic carotid stenosis in patients with coronary heart disease. Results Compared with the non-CAS group, the mean age of the CAS group was (60.04±3.41) years old, and the number of patients with a history of smoking and stroke history was 22, and the difference between the 8 groups was statistically significant (P〈0.05). At the same time, there was a close relationship between atherosclerotic carotid stenosis and atherosclerotic plate incidence. The incidence of CCA atherosclerotic plaque in the CAS group was 64.44%, and the incidence of ICA atheroma was 17.78%. The incidence of CCA atherosclerotic plaque in Non-CAS group was 6.67%, and the incidence of ICA atheroma was 2.22%. The numerical comparison was statistically significant (χ2=8.91, 5.87, P〈0.05). Conclusion The occurrence of atherosclerotic carotid stenosis in patients with coronary heart disease is closely related to age characteristics, disease history and living habits. The incidence of atherosclerotic plaque in patients with atherosclerotic carotid artery stenosis is high and needs to be improved. The importance of clinical screening and risk factor analysis should be paid attention t0.
