急性心肌梗死患者的B型钠尿肽和肌钙蛋白Ⅰ水平的评估价值 被引量:1

摘要 目的研究B型钠尿肽(BNP)和肌钙蛋白Ⅰ(cTnⅠ)的水平对于急性心肌梗死的患者的远期心力衰竭的评估价值。方法选取2017年1月—2018年2月在该院进行治疗的AMI(急性心肌梗死)的患者80例,根据这些患者在进行治疗之后有无并发心力衰竭而将其分为对照组和心衰组,其中对照组63例,心衰组17例。比较这两组患者在入院时的B型钠尿肽(BNP)和入院时以及入院6 h后血清肌钙蛋白Ⅰ(cTnⅠ)的水平。结果在入院时,心衰组患者的B型钠尿肽水平(1 568.28±21.68)pg/mL明显比对照组高,差异有统计学意义(t=171.467,P<0.05);入院即刻对照组和心衰组的血清肌钙蛋白Ⅰ的水平比较,差异无统计学意义(P>0.05);入院6 h心衰组患者的血清中肌钙蛋白Ⅰ的水平(5.23±1.25)μg/L高于对照组,差异有统计学意义(t=14.356,P<0.05)。结论 B型钠尿肽(BNP)和肌钙蛋白Ⅰ(cTnⅠ)的水平高的急性心肌梗死(AMI)患者更容易出现心力衰竭,所以,监测AMI患者的B型钠尿肽水平以及血清中肌钙蛋白Ⅰ的水平,对于评估AMI患者是否会出现心力衰竭的状况具有非常重要的价值。 Objective To investigate the value of B-type natriuretic peptide (BNP) and troponin I (cTnI) in the assessmerit of long-term heart failure in patients with acute myocardial infarction, Methods Eighty patients with AMI (acute myocardial infarction) who were treated in the hospital from January 2017 to February 2018 were enrolled. According to these patients, they were divided into control group and heart failure group according to whether they had concomitant heart failure after treatment, including 63 case in the control group and 17 case in the heart failure group. The levels of B-type natriuretic peptide (BNP) at admission were compared between the two groups of patients and serum troponin I (cTnI) levels at admission and six hours after admission. Results At the time of admission, the level of B-type natriuretic peptide (1 568.28±21.68)pg/mL was significantly higher in the heart failure group than in the control group. The difference was statistically significant (t=171.467, P〈0.05). There was no statistically significant difference in serum tro- ponin I levels in the heart failure group(P〉0.05). Serum troponin I levels (5.23±1.25)μg/L were higher in the six-hour heart failure group than in the control group. Statistically significant (t=14.356, P〈0.05). Conclusion Patients with a- cute myocardial infarction (AMI) with high levels of B-type natriuretic peptide (BNP) and troponin I (cTnI) are more likely to develop heart failure. Therefore, monitoring the level of B-type natriuretic peptide and serum in patients with AMI level of troponin I is of great value in assessing whether AMI patients develop heart failure.
