摘要 目的:探讨超声剪切波弹性成像(SWE)对乳腺癌病理特征的评估价值。方法:原发性乳腺癌患者76例作为乳腺癌组,乳腺腺瘤患者89例作为乳腺腺瘤组。对比两组患者病灶SWE参数以及病灶中增殖、凋亡、侵袭基因表达量的差异,采用相关性分析判断乳腺癌组织Emax水平与基因表达量的相关性。结果:乳腺癌组病灶Emax水平高于乳腺腺瘤组;增殖相关基因Ghrelin、Jab1、galectin-3、TRIM11、RRS1、MTDH mRNA的表达量高于乳腺腺瘤组;凋亡相关基因SFRP2mRNA的表达量低于乳腺腺瘤组,Smo、TPX2mRNA的表达量高于乳腺腺瘤组;侵袭相关基因AEG-1、HERC4mRNA的表达量高于乳腺腺瘤组,HSPC238、TET1mRNA的表达量低于乳腺腺瘤组;相关性分析发现,乳腺癌病灶SWE参数Emax水平与癌细胞的增殖、凋亡、侵袭活力直接相关。结论:乳腺癌组织SWE参数水平与良性病变存在显著差异,具体水平可用于癌细胞恶性程度的判断。 Objective: To explore the value of shear wave elastography (SWE) for evaluating the pathological featuresof breast cancer. Methods: A total of 76 patients with primary breast cancer were selected as the breast cancer group, and 89 patients with breast adenoma during the same period were selected as the breast adenoma group. The differences in lesion SWE parameter as well as the expression of proliferation, apoptosis and invasion genes in the lesions were compared between the two groups of patients, and correlation analysis was used to judge the correlation between Emax level and gene expression in breast cancer tissue. Results: The Emax level in the lesions of breast cancer group was higher than that of breast adenoma group; ex pressions of proliferation-related genes Ghrelin, Jab1, galectin-3, TRIM11, RRS1 and MTDH mRNA in the lesions were high breast adenoma group whereas Stud and TPX2 mRNA ex related genes AEG 1 and HERC4 mRNA expressions in t gene SF pression he lesio lower than that of a group; invasionma group whereas HSPC238 and TET1 mRNA expressions were lower than those of breast adenoma group; correlation analysis showed that the SWE parameter Emax level in breast cancer lesions was directly correlated with the proliferation, apoptosis and invasion activity of cancer cells. Conclusion: SWE parameter level in breast cancer tissue is significantly different from that in benign lesions, and the specific level can be used to determine the malignant degree of cancer cells.
