摘要
目的评价利伐沙班和依诺肝素在预防髋部周围骨折和下肢长骨骨折患者深静脉血栓(DVT)形成的疗效及安全性。方法川北医学院附属医院骨科2015年7月—2017年7月新入院的髋部、下肢长骨骨折手术患者180例,其中90例患者口服利伐沙班10mg qd;另90例患者皮下注射依诺肝素0. 6mL qd。比较两组患者的失血量、围术期凝血指标变化以及DVT发生情况来评价利伐沙班和依诺肝素的疗效及安全性。结果(1)两组患者在年龄、性别、体重指数方面差异无统计学意义(P> 0. 05);(2)两组患者失血总量及围术期凝血指标差异无统计学意义(P> 0. 05);(3)随访6个月,两组患者DVT发生的概率差异无统计学意义(P> 0. 05)。结论利伐沙班与依诺肝素预防髋部周围骨折和下肢长骨骨折患者DVT形成的疗效及安全性并无明显差异,临床上应根据实际情况进行选择及调整。
Objective To evaluate the efficacy and safety of the rivaroxaban and enoxaparin in prevention of deep venous thrombosis in patients with fracture of hip and lower extremities. Methods A total of 180 cases with fracture of hip and lower extremities from Jul. 2015 to the Jul. 2017 in the Department of Orthopedics of Affiliated Hospital of NSMC were enrolled in the present study. Ninety patients were treated with rivaroxaban（10mg qd through oral administration） and the rest patients received enoxaparin （0.6mL qd by hypodermic injection）. The efficacy and safety of these two medicines were evaluated by the comparison of some variations including blood loss,changes of the index of coagulation function during the perioperative period and the rate of DVT between the two groups. Results The demographic characterizations including the ratio of gender,age and the body mass index in patients of the two groups presented no significant difference. What s more,there was no significant difference in the blood loss and the index of coagulation function during the perioperative period as well as the rate of the DVT during a six-month follow-up between the two groups. Conclusion The efficacy and safety of rivaroxaban and enoxaparin presented no obvious difference in preventing deep venous thrombosis in patients with fracture of hip and lower extremities and the clinical application of these two medicines should be adjusted according to patients condition.
