摘要
目的分析传染科手足口病医务人员肠道病毒71型(EV71)和柯萨奇病毒A组16型(CA16)的隐性感染情况。方法 2016年6~9月杭州市儿童医院儿科医务人员63人(其中传染科医务人员23人),健康成人体检43人,采用荧光PCR法进行粪便EV71和CA16病毒核酸检测,利用酶联免疫分析(ELISA)间接法检测血清EV71和CA16的IgG抗体。结果医务人员与非医务人员粪便肠道病毒核酸均阴性。医务人员与非医务人员血清EV71抗体IgG阳性率分别为17.46%及18.60%,差异无统计学意义(χ~2=0.023,P=0.88);血清CA16抗体IgG阳性率分别为95.24%和53.49%,差异有统计学意义(χ~2=26.219,P=0.00)。传染科与非传染科医务人员比较,血清EV71抗体IgG阳性率分别为26.09%和12.50%,差异无统计学意义(χ~2=1.047,P=0.31);血清CA16抗体IgG阳性率分别为91.30%和97.50%,差异无统计学意义(χ~2=0.247,P=0.62);医生与护士比较,血清EV71抗体IgG阳性率分别为11.11%和35.71%,差异无统计学意义(χ~2=1.72,P=0.19);血清CA16抗体IgG阳性率分别为100.00%和92.86%,差异无统计学意义(χ~2=0.672,P=0.41)。结论儿科医务人员CA16隐性感染要高于健康人群。手足口病医务人员虽然存在着一定程度EV71和CA16隐性感染情况,但粪便均未能检测出EV71和CA16病毒核酸,说明传播风险系数较低。另手足口病病房与其他病房医务人员EV71和CA16隐性感染情况无差异。
Objective To understand the status of latent infection of Enterovirus 71 (EV71) and Coxsackie A16 (CA16) among medical workers in Division of Pediatric Infectious Diseases. Methods A total of 63 health care workers (including 23 personnel working specifically for HFMD) and 43 healthy controls were en rolled into the study. Blood and fecal samples were collected to test EV71 /CA16 viral RNA by Real-time RT PCR and IgG antibodies against EV71/CA16 in serum by ELISA. Results All fecal samples were negative for both EVT1 and CA16 viral RNA. The difference of antibody positive rate was statistically significant only for anti-CA16 IgG between medical personnel and healthy controls (95.24G vs 53.49G, χ^2= 26. 219, P= 0.00) not for anti-EV71 IgG (17.46% ws 18.60%)(χ^2=0.023, P=0.88).There were no significant differ ences of serum lgG antibody positive rates for both CA16 and EV71 between health care workers in infectious disease division and non-infectious disease divisions (χ^2=0. 247, 1. 047, P=0.62, 0.31) as well as between doctors and nurses (χ^2=0. 672, 1.72, P= 0.41, 0. 19). Conclusions Although the latent infection rate of CA16 is higher among medical workers than healthy controls, viral RNA is absent in fecal samples, indicating a low transmission risk, even for those working with HFMD.
