摘要
目的探讨V形截骨术治疗拇外翻的临床疗效。方法采用第一跖骨远端V形截骨术治疗拇外翻患者32例,共51足。结果本组共32例(51足)患者。术后28例患者获随访3～25个月,3例(3足)因皮神经损伤导致拇趾麻木;2例(3足)出现足底疼痛;1例(1足)术后外观不满意;1例(1足)第一跖趾关节发生坏死导致截肢。其余患者术后患足外形改善较明显,拇囊红肿消失,行走时疼痛症状消失或明显缓解。结论 V形截骨术对轻、中度拇外翻(拇外翻角<40°)治疗效果较好,可减少和避免并发症的发生。
Objective To investigate the efficacy, operative complications, and precautions in Hallux valgus treated by distal metatarsal chevron osteotomy. Methods Thirty-two consecutive patients with 51 symptomatic hallux valgus were treated by chevron osteotomy on the first distal metatarsal. Results Twenty-eight patients were followed up from 3 to 25 months after operation. The foot shape of the patients improved obviously, the bunions disappeared, the pain symptoms disappeared or were significantly relieved when walking. Postoperative complication included： （1） numbness of the big toe induced by injury of the musculocutaneous nerve（3 patients/3 feet）; （2） plantar heel pain after surgery（2 patients/3 feet）; （3） dissatisfaction with the appearance of the foot after surgery（1 patient/1 foot）; ⑷ joint necrosis on the first metatarsal and amputation（1 patient/1 foot）. Conclusion Chevron osteotomy may be a good choice for mild to moderate hallux valgus, but the surgical indications must be properly selected. Complications can be decreased or avoided through a standardized operation.
