摘要 目的分析血细胞分析仪检测血常规,导致血小板(PLT)假性增多的原因,并寻找解决方法。方法对2例血细胞分析仪检测血小板异常增多的样本,首先血涂片染色镜检,观察破碎红细胞、血小板形态及分布情况,再分别用网织红细胞检测通道采用核酸荧光染色法(PLT-O)和显微镜手工镜检两种方法复检血小板。结果 2例皆为假性血小板增多,PLT-O法与手工镜检法计数结果基本相同。结论在保证血细胞分析仪正常运行的同时,如果检测中发现血红蛋白(HGB)偏低、红细胞平均体积(MCV)偏低(<70 fL)、PLT异常增多的结果,在仪器提示有异常直方图时,应及时分析可能的影响因素,建议应用XE-5000的PLT-O法进行复检,必要时配合显微镜手工镜检,从而使血小板计数结果更准确、可靠。 Objective To analyze the causes of platelet (PLT) pseudocytosis caused by blood routine detected by blood cell analyzer, and to find a solution. Methods For 2 cases of abnormal increase of platelets detected by blood cell analyzer, blood smear was first stained with microscopy to observe the morphology and distribution of broken red blood cells and platelets. Then, nucleic acid fuorescence staining (plt-o)and manual microscopy were used to reexamine platelets in the reticular erythrocyte detection channel. Results Both cases were Pseudo thrombocythemia; PLT-O and manual counting results are basically identical. Conclusion If the blood cell analyzer during its normal running reads a low HGB,low MCV (〈70 fL) or an unusually high PLT, and registers an abnormal histogram,it is supposed to analyze the possible infuence factors and suggested to recheck by PLT-O with XE-5000, and by manual microscopic examination if necessary, so that the counting results of platelet can be more accurate and reliable.
