伴髓样特征的乳腺癌MRI形态学量化分析及信号特征 被引量:3

Quantitive Analysis of Morphology and Signal Characteristics on MRI of Breast Carcinoma with Medullary Features

摘要 目的探讨伴髓样特征的乳腺癌MRI形态学及平扫、动态增强(DCE)扫描、扩散加权成像(DWI)表现特征。方法搜集2005年7月至2016年12月期间于本院行乳腺MRI检查并经手术病理证实的15例伴髓样特征的乳腺癌患者,回顾性分析病变的形态学、平扫T1WI及T2WI信号、DCE表现及时间-信号强度曲线(TIC)类型、DWI信号及表观扩散系数(ADC)值,并引用影像组学中肿瘤球度参数对其形态学进行量化分析,对MRI上病变区进行手动分割,运用Matlab2014b软件提取肿瘤球度参数。结果 15例伴髓样特征的乳腺癌在MRI上均为肿块型病变,86.7%(13/15)呈圆形或卵圆形,平均球度为93.68%(85.5%-98.59%);86.7%(13/15)边缘清晰;73.3%(11/15)于平扫脂肪抑制T2WI信号均匀;DCE后15例早期时相均呈较均匀强化,随时间延迟93.3%(14/15)以边缘强化为著,TIC 66.7%(10/15)呈流出型(Ⅲ型)、33.3%(5/15)呈平台型(Ⅱ型);DWI均呈明显高信号,ADC值均较低。结论伴髓样特征的乳腺癌形态学多呈规则的圆形,MRI平扫T2WI呈均匀的较高信号,DCE后早期时相呈较均匀强化,随时间延迟以边缘强化为著,TIC类型和DWI上ADC值呈典型恶性特征。 Objective To characterize the MRI morphologic and signal features of breast carcinoma with medullary features. Methods MRI images of 15 patients with breast carcinoma with medullary features confirmed by histopathology from July 2005 to December 2016 were analyzed systematically. MRI findings included tumor morphology,signal intensity on plain T1WI and T2WI,appearance of DCE-MRI,the type of time-signal intensity curve( TIC) and DWI features. As a quantitative index,the tumor sphericity measured by Matlab2014b software was used to evaluate the MRI morphology of breast carcinoma with medullary features. Results All 15 tumors presented as a mass on MRI. 86. 7%( 13/15) cases were round or oval in shape and the average tumor sphericity of 15 cases was 93. 68%( range 85. 5% ~ 98. 59%); 86. 7%( 13/15) cases had a circumscribed margin; 73. 3%( 11/15) showed homogeneous signal intensity on T2WI; all 15 tumors showed homogeneous enhancement in the early phase and 93. 3%( 14/15) showed delayed peripheral enhancement in the late phase; TIC were type Ⅲ in 66. 7%( 10/15) cases and type Ⅱ in 33. 3%( 5/15) cases; all cases showed high signal intensity on DWI and the value of apparent diffusion coefficient( ADC) was low. Conclusion On MRI,breast carcinoma with medullary features always appeared as masses with a round shape,and homogeneous hyperintensity on T2WI images.Homogeneous enhancement in the early phase and delayed peripheral enhancement in the late phase was frequently seen on contrast enhancement. The type of TIC and ADC value on DWI revealed typical malignant features.
