摘要
目的探讨超声造影在早期乳腺癌前哨淋巴结诊断中的价值。方法 123例早期乳腺癌患者,术前局部皮下注射声诺维行超声造影,标记前哨淋巴结位置,记录淋巴结及其引流淋巴管的增强表现;乳晕皮下注射纳米炭混悬液,行乳腺癌改良根治+部分腋窝淋巴结清扫术;以手术组织病理检查结果为标准,计算超声造影检测前哨淋巴结的准确率。结果手术切除前哨淋巴结168枚,手术组织病理证实转移淋巴结57枚;超声造影共检出淋巴结132枚,超声造影诊断转移淋巴结45枚;与手术组织病理结果进行比较,超声造影检出前哨淋巴结的准确率为78.6%(132/168),诊断淋巴结转移的准确率为78.9%(45/57)。结论超声造影对早期乳腺癌前哨淋巴结定位及定性诊断有一定价值。
Objective To investigate the value of contrast-enhanced ultrasound to the diagnosis of sentinel lymph nodes in early breast cancer.Methods Totally 123 patients with early breast cancer received subcutaneous injection of sonovue ultrasound contrast before surgery.The locations of sentinel lymph nodes were marked and the enhancement of lymph nodes and their drainage lymphatics were recorded.After the nano-carbon suspension was injected subcutaneously into the areola of breast,partial axillary lymph node dissection plus modified radical resection of breast cancer was performed.The accurate rate of contrast-enhanced ultrasound for sentinel lymph nodes was calculated by comparing with surgical histopathological results.Results A total of 168 sentinel lymph nodes were surgically removed,in which 57 were pathologically confirmed metastatic lymph nodes.Totally 132 lymph nodes were detected by contrast-enhanced ultrasound,including 45 metastatic lymph nodes.Compared with the surgical pathological results,the accuracy rate of contrast-enhanced ultrasound for sentinel lymph nodes was 78.6%（132/168）,and the accuracy for metastatic lymph nodes was 78.9%（45/57）.Conclusion Contrast-enhanced ultrasound has a certain significance in the location and qualitative diagnosis of sentinel lymph node in early breast cancer.
