幽门螺杆菌分型检测及其与幽门螺杆菌根除率的关系研究 被引量:7

摘要 目的:通过应用蛋白芯片技术检测幽门螺杆菌(helicobacter pylori,Hp)血清学分型,探讨Hp血清学分型与其根除率的关系。方法:收集2016-10~2017-10因上消化道症状就诊于盘锦市中心医院,采用蛋白芯片技术进行血清学检测,且接受四联疗法根除治疗的Hp感染患者321例。根据检测结果进行分组,细胞毒素相关蛋白(cytotoxin-associated gene A,CagA)、空泡毒素相关蛋白(vacuolating cytotoxin A,VacA)、尿素酶相关蛋白(urease,Ure)均阳性为Ⅰ型组,CagA(-)、VacA(-)、Ure(+)为Ⅱ型组,CagA(+)、VacA(-)、Ure(+)或CagA(-)、VacA(+)、Ure(+)为中间型组。对所有入选患者进行随访,比较各组Hp根除率,并分析Hp分型与其根除率的关系。结果:321例患者中,失访15例,失访率为4.7%,最终入组患者306例,Ⅰ型、Ⅱ型、中间型患者分别为141例、72例、93例。3组患者根除率分别为95.0%(134/141)、81.9%(59/72)、83.9%(78/93),3组Hp根除率比较,差异具有统计学意义,且Ⅰ型组显著高于Ⅱ型组及中间型组(P<0.01)。阿莫西林、克拉霉素、雷贝拉唑及铋剂四联方案在该地区中的Hp根除率为88.6%。结论:Hp根除率与血清学分型有关,根据Hp分型可指导临床医生做出是否对Hp进行根除治疗的决策。 Objective: To detect the Helicobacter pylori (Hp) serological typing by protein chip technology and to investigate the relationship between the Hp type and it's eradication rate.Methods: A total of 321 patients with upper gastrointestinal symptoms between October 2016 to October 2017 in Panjin central hospital, who were diagnosed with Hp infection by protein chip technology for serological detection and received the eradication treatment with quadruple therapy.All patients were divided into type Ⅰ group,type Ⅱ group and middle type group according to test result. All patients were followed up, the Hp eradicate rate of three groups were compared, and the relationship between Hp type and eradication rate were analyzed.Results=There were 321 patients in which 15 lost to follow--up (4.7%),totally 306 patients were enrolled. The Hp eradication rates in type Ⅰ group, type Ⅱ group and type middle group were 95.0% ( 134/141 ), 81.9% ( 59/72 ), 83.9% ( 78/93 ), respectively. There was significant difference in three groups (P〈0.05).The Hp eradication rates in type I group was higher than that of type Ⅱ group and type middle group, the differences were statistically significant(P〈0.01). The Hp eradication rate in this region by using quadruple therapy (Amoxicillin, Clarithromycin, Rabeprazole, Bismuth ) was 88. 6%. Conclusion. The Hp eradication rate was associated with the serological typing. Therefore, the decision of Hp eradication treatment can be made based on Hp serological typing.
