摘要
目的探讨系统性淋巴结清扫术治疗子宫内膜癌(EC)的预后效果及安全性。方法选择2011年2月-2014年2月郑州人民医院收治的EC患者76例,以随机数字表分组为研究组与对照组,各38例。对照组行子宫全切+双附件切除+盆腔淋巴结清扫手术,研究组行子宫全切+双附件切除+系统性盆腔、腹主动脉旁淋巴结清扫术。通过观察两组EC患者术中指标、术后恢复与随访情况来评价两种术式的预后与安全性。结果研究组患者手术时间长于对照组,淋巴结清扫数量多于对照组(P<0. 05)。研究组术后整体并发症的发生率为23. 68%,低于对照组的50. 00%(χ~2=5. 655,P<0. 05)。随访3年,研究组EC患者复发率为10. 53%,低于对照组31. 58%(χ~2=5. 067,P<0. 05);研究组生存率94. 74%,高于对照组73. 68%(χ~2=5.333,P<0. 05)。结论系统性淋巴结清扫术能够保证EC患者的预后质量,虽然增加了并发症概率,但仍在可控范围内,安全性尚可。
Objective To evaluate the prognosis and safety of systemic lymphadenectomy for endometrial carcinoma（ EC）.Methods 76 patients with EC in our hospital from February 2011 to 2014 February were randomly divided into study group and control group,38 cases in each group.The control group underwent total hysterectomy,double adnexectomy and pelvic lymph node dissection.The study group underwent total hysterectomy,double annexectomy,and systemic pelvic and paraaortic lymph node dissection.The prognosis and safety of the two groups were evaluated by observing the intraoperative indexes,postoperative recovery and follow-up of EC patients.Results The operative time of EC in the study group was longer than that in the control group,and the lymph node from dissection was more than that in the control group（ P0.05）.The incidence rate of postoperative complications in the study group（ 23.68%） was lower than that in the control group（ 50%）（ P0. 05）.After 3 years follow-up,the recurrence rate of EC in the study group（ 10.53%） was lower than that in the control group（ 31.58%）（ P0. 05）,and the survival rate in the study group（ 94.74%） was higher than that in the control group（ 73.68%）（ P0.05）.Conclusion The systemic lymphadenectomy can ensure the prognosis quality of EC patients.Although the probability of complications has been increased,it is still within the controllable range,and the safety is still available.
