摘要
目的观察七叶神安分散片联合地西泮治疗焦虑性失眠的安全性和有效性。方法选取中国人民解放军第一八0医院2014年6月—2017年6月收治的焦虑性失眠患者78例,随机分成对照组(39例)和治疗组(39例)。对照组患者口服地西泮片,5 mg/次,2次/d;治疗组患者在对照组基础上口服七叶神安分散片,1片/次,3次/d。两组患者均连续治疗4周。观察两组患者临床疗效,同时比较治疗前后两组患者匹茨堡睡眠质量指数量表(PSQI)、焦虑自评量表(SAS)和汉密尔顿焦虑量表(HAMA)评分,肿瘤坏死因子-α(TNF-α)、5-羟色胺(5-HT)和多巴胺(DA)水平及不良反应情况。结果治疗后,对照组临床有效率为76.92%,显著低于治疗组的94.87%,两组比较差异具有统计学意义(P<0.05)。治疗后,两组患者PSQI、SAS和HAMA评分均显著降低(P<0.05),且治疗后治疗组PSQI、SAS和HAMA评分明显低于对照组(P<0.05)。治疗后,两组患者TNF-α显著降低(P<0.05),5-HT和DA水平显著升高(P<0.05),且治疗组患者TNF-α、5-HT和DA治疗后水平明显好于对照组(P<0.05)。治疗期间,治疗组不良反应发生率为5.13%,显著低于对照组患者的25.64%,两组患者比较差异具有统计学意义(P<0.05)。结论七叶神安分散片联合地西泮治疗焦虑性失眠疗效好,不良反应发生率低,具有一定的临床推广应用价值。
Objective To investigate the safety and efficacy of Qiye Shenan Dispersible Tablets combined with diazepam in treatment of anxiety insomnia. Methods Patients（78 cases） with anxiety insomnia in the 180 th Hospital of PLA from June 2014 to June 2017 were randomly divided into control（39 cases） and treatment（39 cases） groups. Patients in the control group were po administered with Diazepam Tablets, 5 mg/time, twice daily. Patients in the treatment group were po administered with Qiye Shenan Dispersible Tablets on the basis of the control group, 1 tablet/time, three times daily. Patients in two groups were treated for 4 weeks. After treatment, the clinical efficacy was evaluated, and the PSQI scores, SAS scores, HAMA scores,TNF-α levels, 5-HT levels, DA levels and adverse reactions in two groups before and after treatment were compared. Results After treatment, the clinical efficacy in the control group was 76.92%, which was significantly lower than 94.87% in the treatment group, and there were differences between two groups（P 〈 0.05）. After treatment, the PSQI, SAS and HAMA scores in two groups were significantly decreased（P 〈 0.05）, and these scores in the treatment group after treatment were significantly lower than those in the control group（P 〈 0.05）. After treatment, the TNF-α levels in two groups were significantly decreased（P 〈 0.05）, 5-HT and DA levels were significantly increased（P 〈 0.05）, and the TNF-α, 5-HT and DA levels in the treatment group after treatment were significantly better than those in the control group（P 〈 0.05）. During the treatment, the adverse reactions rate in the treatment group was 5.13%, which was significantly lower than 25.64% in the control group, with significant difference between two groups（P 〈 0.05）. Conclusion Qiye Shenan Dispersible Tablets combined with diazepam in treatment of anxiety insomnia have good effect with lower adverse reactions incidence, which has a certain clinical application value.
