联合应用磁共振动态增强和弥散加权成像诊断进展期胃癌的价值 被引量:6

Value of combined use of MRI dynamic enhancement and diffusion weighted imaging in the diagnosis of advanced gastric cancer

摘要 目的探讨磁共振动态增强联合弥散加权成像在进展期胃癌患者中的诊断价值。方法疑似进展期胃癌患者102例。术前先进行磁共振动态增强扫描,然后行弥散加权成像检查,将其诊断结果与手术病理诊断进行比较。采用ROC曲线分析磁共振动态增强和弥散加权成像在进展期胃癌患者中的诊断效能。结果 102例中,89例病理确诊为进展期胃癌,13例为胃部良性疾病。进展期胃癌患者的转运常数、血管外细胞外间隙体积百分数和速率常数均高于胃部良性疾病患者(P<0.05)。当扩散敏感因子为400和800s/mm^2时,进展期胃癌患者的弥散加权信号强度值及表面弥散系数值均低于胃部良性疾病患者(P<0.05)。联合应用磁共振动态增强和弥散加权成像诊断进展期胃癌的灵敏度和特异度均高于单一方法(P<0.05)。结论联合应用磁共振动态增强和弥散加权成像诊断进展期胃癌的效果较单一方法更为确切。 Objective To explore the value of combined use of MRI dynamic enhancement and diffusion weighted imaging in the diagnosis of advanced gastric cancer.Methods In 102 patients with suspected advanced gastric cancer,MRI dynamic enhancement scanning was conducted before surgery,and then diffusion weighted imaging was done.The results were compared with pathological examination after surgery.The value of MRI dynamic enhancement,diffusion weighted imaging and combined use of both methods in the diagnosis of advanced gastric cancer was analyzed using ROC curve analysis.Results Of 102 cases,89 cases were confirmed pathologically to be advanced gastric cancer and 13 cases were benign gastric diseases.The values of transfer constant,percentage of extravascular extracellular space volume and rate constant were all higher in the patients with advanced gastric cancer than those with benign gastric diseases(P〈0.05).When diffusion sensitive factor was 400 s/mm^2 and 800 s/mm^2,respectively,the values of diffusion weighted signal intensity and apparent diffusion coefficient were lower in the patients with advanced gastric cancer than those with benign gastric diseases(P〈0.05).The sensitivity and specificity of combined use of both methods in the diagnosis of advanced gastric cancer were higher than those of MRI dynamic enhancement or diffusion weighted imaging alone(P〈0.05).Conclusion Combined use of MRI dynamic enhancement and diffusion weighted imaging for diagnosing patients with advanced gastric cancer is more accurate than use of MRI dynamic enhancement or diffusion weighted imaging alone.
