摘要
目的:探讨超声血流参数测量在预测宫内发育迟缓(IUGR)中的应用价值。方法:选取陕西省宝鸡市中心医院2016年5—12月收治的96例IUGR孕妇为观察组,选取同期收治的80例正常孕妇为对照组,两组均行超声血流参数测量。获取胎儿的脐动脉、脐静脉、肾动脉、大脑中动脉等血流频谱参数,比较两组动脉阻力指数(RI)、动脉搏动指数(PI)、收缩期峰值流速与舒张末期流速之比(S/D)、静脉导管血流量(QDV)、脐静脉血流量(QUV)等指标。结果:观察组脐动脉RI、PI以及S/D均高于对照组,大脑中动脉RI、PI以及S/D均低于对照组,肾动脉RI高于对照组,差异有统计学意义(P<0.05)。观察组QUV低于对照组,QDV/QUV、下腔静脉返流分数、QDV高于对照组,差异有统计学意义(P<0.05)。新生儿Apgar评分≤7分组脐动脉及肾动脉RI、PI以及S/D均高于Apgar评分>7分组,大脑中动脉RI、PI以及S/D均低于Apgar评分>7分组,差异有统计学意义(P<0.05)。结论:超声血流参数在评估胎儿发育迟缓及宫内缺氧中有较高的应用价值,可结合参数进行预测。
Objective:To explore the application value of ultrasound blood flow parameters in prediction of intrauterine growth retardation(IUGR).Methods:Ninety-six pregnant women with IUGR in our hospital from May to December 2016 were selected as an observation group.Eighty normal pregnant women in the same period were selected as a control group.Ultrasound blood flow parameters were measured in both groups.Fetal umbilical artery,umbilical vein,renal artery,middle cerebral artery and other blood spectrum parameters were obtained,and the RI,PI,S/D,QDV and QUV were compared.Results:The RI,PI and S/D of the umbilical artery as well as the RI of the renal artery in the observation group were higher than those in the control group,while the RI,PI and S/D of the middle cerebral artery were lower(P〈0.05).The QUV in the observation group was lower than that in the control group,and the QDV/QUV,reflux fraction and QDV in inferior vena cava were higher(P〈0.05).The RI,PI and S/D of the umbilical artery and renal artery in newborns with Apgar score≤7 were higher than those with Apgar score〉7,and the RI,PI and S/D in the middle cerebral arteries were lower(P〈0.05).Conclusion:Ultrasound blood flow parameters was valuable in evaluating fetal growth retardation and intrauterine hypoxia.
