阴道锥体训练联合生物反馈盆底肌治疗尿失禁的临床效果及对患者尿动学参数的影响 被引量:7

Clinical effects of vaginal cone training combined with biofeedback pelvic floor muscle therapy on urinary incontinence and urodynamic parameters

摘要 目的探讨在压力性尿失禁患者中采用阴道锥体训练联合生物反馈盆底肌治疗后的效果及尿动学变化。方法选取暨南大学附属珠海医院2017年1月至2018年2月114例压力性尿失禁患者,并随机分为两组,对照组共56例,采用常规的盆底肌训练;观察组58例,采用阴道锥体训练联合生物反馈盆底肌治疗,比较两组患者的临床治疗效果,尿动学参数腹压漏尿点压(VLPP)、最大尿道闭合压(PMUC)、生活质量及盆底肌状况。结果治疗后观察组中治愈的患者显著多于对照组(87.9%vs.50.0%),观察组的生活质量评分也明显高于对照组(P<0.05),观察组经过治疗后尿动学参数VLPP、PMUC显著高于对照组,观察组的盆底肌力等级中Ⅳ级和Ⅴ级患者显著多于对照组(P<0.05)。结论在尿失禁患者中采用阴道锥体训练联合生物反馈盆底肌治疗具有良好的治疗效果,值得临床推广和应用。 Objective To investigate the clinical effects and changes of urodynamics after vaginal cone training combined with biofeedback therapy was applied in patients with urinary incontinence. Methods A total of 114 patients with urinary incontinence treated during Jan.2017 and Feb.2018 were randomly divided into two groups:control group ( n =56, received routine pelvic floor muscle training), and observation group ( n =58, received vaginal cone training combined with biofeedback pelvic floor muscle therapy).The clinical outcomes of the two groups were compared, including changes in Valsalva leak point pressure (VLPP), pressure of maximum urethral closure (PMUC), quality of life, and pelvic floor muscle status. Results After treatment, more patients were cured in the observation group than in the control group (87.9 % vs. 50.0 % ).The quality of life, VLPP, and PMUC were significantly higher in the observation group than in the control group ( P 〈0.05).There were more patients with grade Ⅳ and Ⅴ pelvic floor muscle strength in the observation group than in the control group ( P 〈0.05). Conclusion Vaginal cone training combined with biofeedback pelvic floor muscle therapy has a good therapeutic effect for patients with urinary incontinence, which is worthy of clinical promotion and application.
