摘要
目的分析讨论骨科重症创伤患者血清白介素-6、肿瘤坏死因子-α以及高敏C反应蛋白水平联合检测的临床价值。方法本研究选取骨科重症创伤患者97例纳入研究,收集测定受试对象创伤初诊时、伤后3天以及伤后7天的血清IL-6、TNF-α以及hs-CRP水平,分析总结上述指标的表达和变化特征,同时收集随访1年血栓预后的检出情况及其与各指标之间联合检测的相互关联。结果本次研究中,纳入的97例受试对象不同时间点患者的IL-6血清水平平均值以及增高者构成比例差别有统计学意义,P<0. 05。血清TNF-α水平变化趋势与IL-6类似,血清hs-CRP水平变化趋势亦如此。进一步分析了受试对象IL-6、TNF-α及hs-CRP水平的相互关系及与预后的关联。受试对象IL-6、TNF-α及hs-CRP水平的联合检测与血栓预后的关联的多因素Logistic回归分析结果提示,初诊、伤后3天以及伤后7天的IL-6、TNF-α及hs-CRP三项均增高是发生血栓预后的危险因素,OR值分别为2.218、3.787和2. 100。结论血清白介素-6、肿瘤坏死因子-α、高敏C反应蛋白水平变化,初诊时3项指标均可见较高表达,且伤后3天水平继续增高,7天后水平有所回落。同时3个指标联合检测对于血栓预后的发生具有重要临床预测价值。
Objective To analyze and discuss the clinical value of joint detection of serum intedeukin-6, tumor necrosis factor-α and high-sensitivity C-reactive protein in patients with severe trauma of orthopedics. Methods 97 patients with severe traumatic orthopedics were enrolled in this study. Serum levels of IL-6, TNF-α and hs-CRP were measured at the first visit, 3 days after injury and 1 week after injury. The changes of thrombosis status and follow-up were also analyzed. The detection of thrombosis prognosis and its correlation with the joint detection of each index were collected at the same time. Results In this study, the mean values of the IL-6 serum levels and the proportions of the increased components of the 97 subjects included at different time points were statistically significant, P 〈 0.05. The trend of serum TNF- c~ levels was similar to that of IL- 6, and the trend of serum hs- CRP levels was also changed. The relationship among the levels of IL-6, TNF-α and hs-CRP in the subjects and their association with prognosis were further analyzed. Multivariate logistic regression analysis of the association of IL-6, TNF-α, and hs-CRP levels with the prognosis of the thrombus in the subjects suggested that IL-6 and TNF at 7 days after the first visit and 3 days after injury The increase of both -alpha and hs-CRP was a risk factor for the prognosis of thrombosis. The OR values were 2. 218, 3. 787, and 2. 100, respectively. Conclusion Serum levels of interleukin-6, tumor necrosis factor-a and high-sensitivity C-reactive protein significantly increase in patients with severe trauma in orthopedics. Three indicators of patients are highly expressed in the first visit, and the levels continue to increase to a higher level on the third day after the last visit. After 1 week, the level hasdropped back. At the same time, the joint detection of three indexes has important clinical predictive value for the occurrence of thrombus prognosis.
