急性冠脉综合征不同病程、不同疾病类型患者YKL-40、IL-35、脂联素等表达特征分析 被引量:8

A Characteristic Analysis of Expression of YKL-40,IL-35 and Adiponectin Among Patients with Different Degrees and Types of Acute Coronary Syndrome

摘要 目的分析急性冠脉综合征(ACS)具体疾病类型及不同病变程度与患者血清几丁质酶-3样蛋白1(YKL-40)、白细胞介素-35(IL-35)、超敏C反应蛋白(hs-CRP)、脂联素表达水平的特征及相关性。方法自本院2015年6月至2017年1月间收治的ACS患者中选取90例作为疾病组,其中包括急性心肌梗死(AMI)患者30例、不稳定型心绞痛(UAP)30例及稳定型心绞痛(SAP)30例,以三种疾病作为三个疾病亚组;选取同期本院收治的胸部疼痛但冠状动脉造影显示正常的30例患者作为对照组;对比疾病组与对照组间血糖、血脂指标;观察组三疾病亚组与对照组间血清指标的表达特征。根据Gensini积分将疾病组患者按病变程度划分为轻度、中度、重度三亚组,观察三亚组间血清指标的表达特征。分析血清指标与ACS患者血脂、血糖,病情程度及疾病类型间的相关性。结果疾病组患者TG、TC、空腹血糖水平均高于对照组,P<0.05;三疾病亚组与对照组间hs-CRP、IL-35、YKL-40、脂联素表达水平差异均具有统计学意义,P<0.05;不同病变程度亚组组间hs-CRP、IL-35、YKL-40、脂联素表达水平差异均具有统计学意义,P<0.05。hs-CRP、IL-35、YKL-40、脂联素表达水平与病情程度间具有高度相关性,|r|≥0.8,<0.05;与TC、TG、空腹血糖水平间均具有高度相关性,|r|≥0.8,P <0.05;hs-CRP、脂联素与ACS疾病分型间具有中度相关性,0.5≤|r|<0.8,IL-35、YKL-40与ACS疾病分型间具有高度相关性,|r|≥0.8,P<0.05。结论急性冠脉综合征患者血清YKL-40、IL-35、hs-CRP、脂联素与病情程度、疾病类型及血脂、血糖间均具有相关性,各项指标的表达水平可为ACS的病程评估及疾病类型的甄别提供重要参考。 Objective To analyzedifferent disease types and degrees of lesions in patients with acute coronary syndrome (ACS) and their correlations withserum levels of chitinase- 3 like protein 1 ( YKL-40 ), interleukin 35 ( IL- 35 ), high- sensitivity C- reactive protein ( hs- CRP), and adiponectin. Methods 90 patients with ACS who were admitted to our hospital from June 2015 to January 2017 were selected as the disease group, including 30 patients with acute myocardial infarction ( AMI), 30 patients with unstable angina (UAP) and 30 patients with acute stable angina pectoris (SAP), which served as three disease subgroups. 30 patients with chest pain but normal coronary angiography admitted to our hospital during the same period were selected as the control group. The blood glucose and lipids indexes were compared between the disease group and the control group, and the expression characteristics of the serum markers among the three disease subgroups and the control group were also compared. According to the Gensini score and the degree of lesions, patients in the disease group were divided into mild, moderate and severe subgroups, and the expression characteristics of serum markers in the three subgroups were observed. The correlations between serum markers and serum lipids ,blood glucose, severity of illness, and disease type in ACS patients was analyzed. Results The levels ofTG,TC and fasting blood glucose in the disease group were higher than those in the control group (P 〈 0.05). There were statistical differencesfor the expression levels of hs-CRP,IL-35 ,YKL-40 and adiponectin between the three disease subgroups and the control group, P 〈 0.05. The expression levels of hs- CRP, IL- 35 ,YKL-40 and adiponectin in the subgroups with different degree of lesions were significantly different,P 〈 0.05.
