无痛胃镜不同镇静方式安全有效性研究 被引量:4

摘要 目的比较分析不同无痛胃镜镇静方式安全性及镇静疗效。方法将2017年2月-2017年4月在武汉市第三医院行无痛胃镜检查患者450例随机分为150例A组(丙泊酚)、150例B组(丙泊酚联合咪达唑仑)及150例C组(丙泊酚联合瑞芬太尼)。比较三组心率、血压、血氧饱和度变化,不适反应,苏醒时间和操作时间。结果 B、C组患者的生命体征较A组更为平稳(P<0. 05),不适反应例数显著低于A组(P<0. 05)。B、C组患者的丙泊酚用量较A组更少(P<0. 05),三组操作时间差异无统计学意义(P> 0. 05)。苏醒时间C组<B组<A组(P<0. 05)。结论丙泊酚联合应用咪达唑仑或瑞芬太尼在无痛胃镜的镇静中是安全有效的,相较于单用丙泊酚,联合用药更有利于患者生命体征平稳,减少患者不适,降低丙泊酚用量。 Objective To compare the safety and sedative effects of different sedative methods of painless gastroscopy. Methods From February 2017 to April 2017, 450 patients taken painless gastroscopy in Wuhan No.3 Hospital were randomly divided into three groups, 150 patients in group A (propofol) , 150 patients in group B (propofol combined with midazolam) and 150 patients in group C (propofol combined with remifentanil). The heart rate, blood pressure, oxygen saturation changes, uncomfortable reaction, wake-up time and operation time were compared in three groups. Results The vital signs of patients in group B and C were more stable than those in group A ( P〈0.05 ), and the case number of uncomfortable reaction was significantly lower than that in group A ( P〈0.05 ). The dosage of propofol in group B and C was less than that in group A ( P〈0.05) , and the operation time in the three groups was not statistically significant (P〉0. 05). The wake-up time was group C 〈 group B〈 group A (P〈0. 05). Conclusion Propofol combined with midazolam or remifentanil is safe and effective in the treatment of painless gastroscopy. Compared with propofol alone, the combination therapy is more beneficial to patients with stable vital signs and less discomfort,it can reduce the dosage of propofol.
