摘要
目的比较两种粘度水泥对重度骨质疏松胸腰椎体骨折老年患者经皮椎体成形术(percutaneous vertebroplasty,PVP手术)效果的影响。方法选取2013年1月~2017年1月期间我院收治的82例重度骨质疏松胸腰椎体骨折老年患者为研究对象,所有患者均经PVP手术治疗。依据随机数字表法将患者分为观察组(n=41)和对照组(n=41)。观察组患者术中采用高粘度骨水泥治疗,对照组患者术中采用低粘度骨水泥治疗。比较两组患者Cobb角、伤椎椎体高度和骨水泥渗漏情况。结果治疗前两组患者的Cobb角、伤椎前缘高度百分比、伤椎后缘高度百分比差异无统计学意义(P> 0. 05)。治疗后两组患者的Cobb角较治疗前均降低,伤椎前缘高度百分比和伤椎后缘高度百分比均升高,但观察组患者的Cobb角低于对照组患者,伤椎前缘高度百分比和伤椎后缘高度百分比高于对照组患者,差异具有统计学意义(P <0. 05)。观察组和对照组患者骨水泥总渗漏率分别为7. 32%和21. 95%,对照组显著高于观察组,差异具有统计学意义(P <0. 05)。结论高粘度骨水泥应用于PVP手术中较低粘度骨水泥能够更有效地纠正后凸Cobb角,改善伤椎高度,降低骨水泥渗漏发生率,可在临床上进一步推广和使用。
A total of 82 elderly patients with severe osteoporotic thoracolumbar fractures admitted in our hospital from January 2013 to January 2017 were enrolled in this study. All patients were treated with PVP. Patients were divided into the observation group (n=41) and the control group (n=41) according to the random number table method. Patients in the observation group were treated with high-viscosity cement in the operation, and patients in the control group were treated with lower-viscosity cement in the operation. The Cobb angle, vertebral body height and bone cement leakage rates were compared between the two groups. Results There was no significant difference in the Cobb angle, the percentage of the height of the anterior margin of the injured vertebrae, and the percentage of the height of the posterior margin of the injured vertebrae ( P 〉0.05). After treatments, the Cobb angle decreased, and the percentage of the height of the injured anterior margin and the percentage of the height of the posterior margin of the injured vertebrae increased in both two groups. But the Cobb angle of the observation group was lower than that of the control group, and the percentage of the height of the anterior margin and the posterior margin of the injured vertebra was higher than that of the control group, and the difference was statistically significant ( P 〈0.05). The total leakage rate of bone cement in the observation group and the control group was 7.32% and 21.95%, respectively, and the control group was significantly higher than the observation group. The difference was statistically significant ( P 〈0.05). Conclusion The high-viscosity bone cement used in PVP surgery can effectively correct the kyphosis Cobb angle, improve the height of the injured vertebrae, and reduce the incidence of bone cement leakage, which can be further promoted and used in clinical practice.
