摘要
目的探讨复方血栓通胶囊对2型糖尿病并发糖尿病肾病(DN)患者血浆趋化素和血管内皮生长因子(VEGF)水平的影响。方法选择2015年1—12月间在黄冈市中心医院就诊的DN患者124例,按照随机数字表法分为观察组及对照组,每组62例。对照组在控糖基础上应用缬沙坦口服治疗,观察组在对照组基础上应用复方血栓通胶囊口服治疗,对比治疗前后血糖、肾功能、血液流变学指标及血浆趋化素、VEGF水平的变化。结果观察组治疗后空腹血糖[(5.82±0.71)mmol·L^(-1)]、糖化血红蛋白(7.22%±0.82%)、胰岛素抵抗指数(4.29±0.56)水平低于对照组(P<0.05),而空腹胰岛素[(13.98±3.02)U·L^(-1)]水平高于对照组(P<0.05)。观察组治疗后血肌酐[(86.23±14.57)μmol·L^(-1)]、尿素氮[(4.30±1.21)mmol·L^(-1)]均低于对照组(P<0.05)。观察组治疗后全血高切黏度[(5.12±0.72)m Pa·s^(-1)]、全血低切黏度[(7.42±0.52)m Pa·s^(-1)]、纤维蛋白原[(2.58±0.47)g·L^(-1)]、血浆比黏度[(1.12±0.47)m Pa·s^(-1)]水平显著低于对照组(P<0.05)。观察组治疗后血浆趋化素[(52.22±12.22)μg·L^(-1)]、VEGF[(15.22±4.02)ng·L^(-1)]水平明显低于对照组(P<0.05)。结论复方血栓通胶囊可使DN患者血浆趋化素、VEGF水平明显下降,改善血液黏稠度,提高治疗效果。
Objective To investigate the effect of compound xueshuantong capsule on plasma chemerin and vascular endothelial growth factor( VEGF) in patients with type 2 diabetic nephropathy( DN). Methods A hundred and twenty-four patients of DN treated in Huanggang Central Hospital from January 2015 to December 2015 were randomly assigned into observation group( n = 62) and control group( n = 62) according to random number method. The control group was treated with valsartan on the basis of sugar control and the observation group was treated with compound xueshuantong capsule in addition to the therapy used in the control group. The changes of blood glucose,renal function,hemorheological indexes,plasma chemerin and VEGF before and after treatment were compared. Results After treatment in the observation group,fasting blood glucose [( 5. 82 ± 0. 71) mmol · L^-1],glycosylated hemoglobin( 7. 22% ±0. 82%),and insulin resistance index( 4. 29 ± 0. 56) were lower than those in the control group( P〈0. 05). The level of fasting insulin [( 13. 98 ± 3. 02) U·L^-1]was higher than that of the control group( P〈0. 05). After treatment in the observation group,serum creatinine [( 86. 23 ± 14. 57) μmol·L^-1]and urea nitrogen [( 4. 30 ± 1. 21) mmol·L^-1]were lower than those in the control group( P〈0. 05). After treatment in the observation group,the whole blood high-cut viscosity [( 5. 12 ± 0. 72) mPa·s^-1,whole blood lowcut viscosity [( 7. 42 ± 0. 52) mPa·s^-1,fibrinogen [( 2. 58 ± 0. 47) g·L^-1] and the level of plasma specific viscosity [( 1. 12 ±0. 47) mPa·s^-1were significantly lower than those of the control group( P〈0. 05). After treatment in the observation group,the levels of plasma chemerin [( 52. 22 ± 12. 22) μg·L^-1]and VEGF [( 15. 22 ± 4. 02) ng·L^-1]were significantly lower than those in the control group( P〈0. 05).
