摘要
目的了解该院44例耐多药肺结核治疗后转归情况并对其影响因素进行分析,为耐多药肺结核的防治提供策略。方法选择2016年6月—2017年6月在该院收治的44例结核分枝杆菌耐多药患者,随访所有患者的转归。参考文献自制量表对可能影响耐多药肺结核患者转归的因素进行调查,根据随访结果分为两组,治愈组及预后不良组,后者包括,治疗失败及死亡的患者,通过单因素及多因素分析影响耐多药肺结核患者转归的因素。结果通过单因素分析发现,两组患者在年龄、病变范围、是否规范治疗、化疗药是否合理方面,差异有统计学意义(P<0.05),多因素分析发现:病变范围(OR=1.067,95%CI:1.016~2.286,P=0.001),是否规范治疗(OR=1.060,95%CI:1.387~6.008, P=0.005),化疗药是否合理(OR=6.839,95%CI:1.392~33.596,P=0.018),年龄(OR=1.647,95%CI:0.356~2.176,P=0.034)是影响预后的主要因素。结论年龄、病变范围为不可控因素,因此不规范治疗及使用化疗药不合理适应的合理控制是影响预后的关键。
Objective To know the influencing factors of outcome of patients with multi drug resistant pulmonary tuberculosis treatment thus providing strategies for the prevention and control of multi drug resistant pulmonary tuberculosis.Methods 44 cases of patients with mycobacterium tuberculosis admitted and treated in our hospital from June 2016 to June 2017 were selected, and the outcome of all patients was followed up, and the potential factors of outcome of multi drug resistant pulmonary tuberculosis treatment were surveyed by the self-made scale, and the patients were divided into two groups according to the follow-up results, the cure group and the bad prognosis group(patients with failure treatment and death patients), and the outcome influencing factors were analyzed by the single factor and multi-factor analysis. Results The single-factor analysis showed that the differences in the age, lesion range, standardized treatment or not, whether the chemotherapy drugs were rational or not between the two groups were obvious(P0.05), the multifactor analysis showed that the lesion range(OR=1.067, 95%CI:1.016-2.286,P=0.001), standardized treatment or not(OR=1.060, 95%CI:1.387-6.008,P=0.005), whether the chemotherapy drugs were rational or not(OR=6.839, 95%CI:1.392-33.596, P=0.018) and age(OR=1.647, 95%CI:0.356-2.176, P=0.034) were the major influencing factors of prognosis. Conclusion The age and lesion range are the uncontrollable factors, therefore, the non-standardized treatment and rational control of chemotherapy drug use are the key to influencing the prognosis.
