摘要
目的探讨人工全膝关节置换(total knee arthroplasty,TKA)术后综合康复治疗对膝关节功能恢复的意义。方法选取石家庄市第一医院2015年5月—2017年10月行TKA术患者40例,采用随机数字表法分为观察组和对照组各20例,两组均常规行TKA术,对照组术后给予常规治疗,观察组在此基础上术后给予雷火灸、中药贴敷及下肢主动功能锻炼等综合康复治疗。两组均14 d为1个疗程。观察比较两组术前及术后2周、术后6周美国纽约特种外科医院膝关节评分(HSS),术前及术后1周、2周视觉模拟评分(VAS),以及治疗安全性。结果两组术后2、6周时HSS评分均高于术前,差异有统计学意义(P均<0. 05);且术后2、6周时观察组HSS评分均高于对照组,差异有统计学意义(P均<0. 05)。两组术后1、2周时VAS评分均较术前改善(P均<0. 05);且术后1、2周时观察组VAS评分均低于对照组,差异有统计学意义(P均<0. 05)。两组术后均无明显不良反应发生。结论 TKA术后综合康复治疗能显著减轻患者术后疼痛,改善患者术后不适症状,促进膝关节功能恢复。
Objective To investigate the significance of comprehensive rehabilitation treatment in knee joint function recovel7 after total knee arthroplasty (TKA). Methods A total of 40 patients undergoing TKA fiom May 2015 to October 2017 in the First hospital of" Shijiazhuang were randomly divided into the observation group and the control group, with 20 patients in each group. TKA was pertbmled conventionally in both groups. The control group received conventional postoperative treatment. Based on the control group, the observation group was given the comprehensive rehabilitation treatment of thunder fire moxibustion, Chinese medicine sticking and active functional exer- cise of lower limbs after operation. The course of treatment for both group was 14 days. Hospital for Special Smgery (HSS) score was used to evaluate the function of knees before operation, and at 2 and 6 weeks after operation. VAS was used to evaluate the level of pain before operation, and at 1 and 2 weeks after operation, and treatment safety. Results HSS scores were higher in both groups at 2 and 6 weeks after surgery than those before surgery ( P 〈 0.05 ). The HSS score of the observation group was higher than that of the control group at 2 weeks and 6 weeks after surgery ( P 〈 0.05 ). VAS scores of the two groups were improved at 1 and 2 weeks after surgery (P 〈 0.05 ). VAS score of observation group was lower than that of control group at 1 week and 2 weeks after surgery( P 〈 0.05 ). There was no significant adverse reaction in both groups. Conclusion Comprehensive rehabilitation treatment after TKA can significantly reduce postoperative pain, improve postoperative discombrt and promote functional recovery of knee joints.
