摘要
背景:胃癌是我国常见的恶性肿瘤之一,早期诊断和早期治疗是提高患者生存率的关键。目的:评估复合分光染色(CBI)技术在早期胃癌内镜诊断中的应用价值。方法:2017年1月—2017年12月在上海交通大学医学院附属仁济医院行普通白光内镜检查发现局灶性胃黏膜病变的50例患者纳入研究,在国产CBI内镜下仔细观察病变黏膜腺管形态和微血管结构变化。以手术病理诊断结果为金标准,评估CBI内镜对早期胃癌的诊断效能。结果:50例病灶中29例在CBI模式下诊断为早期胃癌,21例为非早期胃癌。16例患者手术病理诊断为早期胃癌。CBI内镜诊断早期胃癌的敏感性、特异性和准确性分别为93. 8%(15/16)、82. 4%(28/34)和86. 0%(43/50),阳性预测值和阴性预测值分别为71. 4%(15/21)和96. 6%(28/29)。结论:国产CBI内镜对早期胃癌具有良好的诊断价值和临床应用前景,使在基层医疗机构开展早期胃癌筛查成为可能。
Background： Gastric cancer is one of the most common malignancies in China. Early diagnosis and treatment are critical for improving the survival of patients. Aims： To assess the value of compound band imaging （CBI） endoscopy in the diagnosis of early gastric cancer. Methods： Fifty patients with focal gastric mucosal lesion detected by conventional white-light endoscopy from Jan. 2017 to Dec. 2017 at Renji Hospital, School of Medicine, Shanghai Jiao Tong University were enrolled. The morphology and microvascular structure of the mucosal lesion were carefully observed with the CBI endoscopy （domestic made）. Using surgical pathology as gold standard, the diagnostic performance of CBI endoscopy for early gastric cancer was analyzed. Results： Among the 50 cases of focal mucosal lesion, 29 were diagnosed as early gastric cancer and 21 as non-early gastric cancer by CBI endoscopy; 16 cases were proved to be early gastric cancer by surgical pathology. The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of CBI endoscopy for early gastric cancer were 93.8% （15/16）, 82.4% （28/34）, 86.0% （43/50）, 71.4% （15/21） and 96.6% （28/29）, respectively. Conclusions： CBI endoscopy （domestic made） has high accuracy for diagnosis of early gastric cancer and the prospects for future use in clinical practice. The use of CBI endoscopy might make possible the early gastric cancer screening in basic medical and health institutions.
