摘要
目的通过质量持续改进的实施与效果评价,减少医务人员血源性职业暴露的发生率。方法应用PDCA工具进行医务人员血源性职业暴露的现状分析、执行改进措施、严格检查,比较PDCA实施前后职业防护质量考核合格率、血源性职业暴露发生率及不同岗位血源性职业暴露情况。结果实施PDCA后医务人员掌握标准预防概念及上报流程、职业防护箱配置及落实标准预防及隔离措施的合格率均高于实施前,差异有统计学意义(P<0.05);实施PDCA后共发生血源性职业暴露278例次,暴露发生率2.91/100FTE低于实施前(χ~2=6.774,P=0.009);不同岗位血源性职业暴露中护士发生率最高,占39.21%(231/589),其次为学生30.73%(184/589)。结论应用PDCA工具,采取预防为主、注重过程管理,能减少血源性职业暴露的发生,保护医务人员职业安全。
OBJECTIVE To implement the continuous quality improvement and evaluate the effect so as to reduce the incidence of bloodborne occupational exposure among health care workers.METHODS By means of PDCA,the current status of bloodborne occupational exposure among the health care workers was analyzed,the improvement measures were carried out,and rigid examination was conducted.The qualified rate of assessment of quality of occupational protection,incidence of bloodborne occupational exposure and incidence of bloodborne occupational exposure in different positions were observed and compared before and after the PDCA was carried out.RESULTS The qualified rates of the health care workers′awareness of standard prevention concepts and reporting procedures,allocation of occupational protection box and isolation measures were significantly higher after the PDCA was carried out than before it was carried out(P〈0.05).Totally 278 case-times of health care workers had the bloodborne occupational exposure after the PDCA was carried out,the incidence of the exposure was 2.91/100 FTE,significantly lower than that before the PDCA was carried out(χ2=.774,P=0.009).The nurses were the predominant health care workers who had the bloodborne occupational exposure,accounting for 39.21%(231/589),followed by the students 30.73%(184/589).CONCLUSIONIt is an effective way to carry out the PDCA,take the prevention measures as first priority and focus on the process of management so as to reduce the incidence of the bloodborne occupational exposure and ensure the occupational safety of the health care workers.
