纤维支气管镜介入治疗联合化学疗法对耐多药肺结核疗效的meta分析 被引量:8

Meta-analysis of the efficacy of combined interventional therapy through bronchoscope with general chemotherapy in the treatment of multidrug-resistant tuberculosis

摘要 目的评价纤维支气管镜介入治疗联合化学疗法对耐多药肺结核的疗效。方法计算机检索PubMed、EMBASE、Cochrane Library、中国知网、万方数据库和维普数据库,获得纤维支气管镜介入治疗耐多药肺结核的随机对照临床研究,检索时限均从建库至2017年11月30日。由2名研究者独立行文献筛选、资料提取和质量评价,采用RevMan5.3软件进行meta分析并评价纳入研究的偏倚风险。结果最终纳入3388例,其中介入治疗联合化学治疗(试验组)1745例,普通化学疗法(对照组)1643例。介入治疗联合化学治疗在治疗耐多药肺结核中,3个月时痰菌阴转率,疗程结束时痰菌转阴率、病灶吸收显效率、空洞闭合率等与对照组比较差异均有统计学意义。结论介入治疗联合化学治疗明显优于单纯化学疗法。但本评价纳入研究质量有限,尚需开展更多高质量的研究进一步分析。 Objective To evaluatethe efficacy of interventiona) therapy through bronchoscope in the treatment of multidrug-resistant tuberculosis. Methods Searching the PubMed, EMBASE, Cochrane Library, CNKI, Wanfang database and VIP database, to collect randomized controlled trails (RCTs), which all published in the openly journals either in Chinese or English from their establishment to 30th November 2017, and involving interventional therapy through bronchoscope for multidrug-resistant pulmonary tuberculosis. After making data extraction and quality assessment, the RevMan 5.3 software was used to perform meta-analysis and evaluate the risk of bias in the study. Results There were eventually included in 3 388 cases, including 1 745 cases of interventional therapy (experimental group) and 1 643 cases of normal chemotherapy (control group). The results showed in the experimental group, there were significant differences in the rate of sputum negative conversion in the control group at 3 months and the sputum negative conversion rate, the lung lesion absorption rate and curable rate of cavity at the end of treatment were all significantly different between two groups, respectively ( P 〈 0.05). Conclusions Interventional therapy combined with chemical therapy is obviously better than chemotherapy alone. However, the quality of this evaluation is limited, and more high quality research needs to be carried out.
