摘要
目的了解河南省初中生二手烟的暴露情况及影响因素,为制定相应的控烟政策提供依据。方法采取多阶段分层整群随机抽样的方法,采用全球青少年烟草调查问卷,在河南省抽取16个调查点,48所初中,共7789名在校学生进行问卷调查。结果过去7 d内,70.8%的学生报告自己暴露于二手烟,男生高于女生(χ~2=56.65,P<0.001),城市高于农村(χ~2=3.92,P=0.048),不同年级之间差异有统计学意义(χ~2=35.85,P<0.001)。室外公共场所二手烟暴露率最高(57.0%),家中次之(43.0%),公共交通工具最低(15.8%)。多因素Logistic回归分析结果显示,父母吸烟(OR=2.276,95%CI:1.936～2.676)、朋友吸烟(OR=2.343,95%CI:1.876～2.925)、老师吸烟(OR=2.013,95%CI:1.718～2.357)的学生更容易发生二手烟暴露。结论河南省初中学生二手烟暴露情况较为严重,建议尽快推进公共场所控烟立法,有效减少青少年二手烟暴露。
Objective To understand the status of secondhand smoke （SHS） exposure and its influence factors a- mong students in junior schools in Henan Province, and provide scientific basis for corresponding tobacco control policy. Methods 7789 junior school students from 16 districts in Henan Province were chosen by stratified cluster sampling methods and investigated with a questionnaire of Global Youth Tobacco Survey （GYTS）. Results During the past 7 days, 70. 8% of respondents were exposed to SHS. The rate of boy was higher than that of girl （ X2 = 56. 65, P 〈 0. 001 ） , the rate of urban area was higher than that of rural area （X2 = 3.92, P = 0. 048）. The rates of SHS exposure in different grades had significant difference （ X2 = 35.85, P 〈 0. 001 ）. The proportion of SHS exposure in outdoor public places was the highest （57.0%）, followed by at home （43.0%） and in public transport （15.8%）. Multivariate logistic regression analysis demonstrated that the students whose parents （OR=2. 276, 95% CI： 1. 936 -2. 676）, friends （OR = 2. 343, 95% CI： 1. 876 - 2. 925 ） and teachers （ OR = 2.013, 95 % CI： 1.718 - 2. 357 ） smoked were more likely to suffer from SHS exposure. Conclusion The situation of SHS exposure among junior school students in Henan is serious. It is suggested that the laws of prohibiting smoking in public places should be established as soon as possible to reduce teenagers＇ secondhand expo- sure effectively.
