摘要
目的探讨全程系统化干预在腹腔镜下肾癌根治术患者围手术期中的具体应用及效果。方法选取腹腔镜下肾癌根治术患者64例,随机分为对照组与研究组,每组32例。对照组采用传统护理,研究组在传统护理的基础上采用全程系统化护理,对比两组术后躯体康复与心理康复、患者满意度以及生活质量等指标变化。结果研究组术后肛门排气时间、导尿管拔除时间、首次活动时间以及平均住院天数、平均睡眠时间,P均<0.05;干预后两组汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)评分比较,P<0.05;干预后两组健康促进生活方式量表-Ⅱ(HPLP-Ⅱ)比较,P<0.05。结论全程系统化干预可提高腹腔镜下肾癌根治术患者的疗效,减轻疼痛,缓解抑郁焦虑情绪。
Objective To explore the application and effect of systematic nursing intervention in the perioperative period of laparoscopic radical nephrectomy patients with renal carcinoma. Methods 64 laparoscopic radical nephrectomy patients were radomly divided into the control group and the observation group, 32 cases in each group. The control group adopted traditional nursing and the observation group adopted systematic nursing intervention on the basis of traditional nursing. The physical and psychological rehabilitation situtation, patients＇ satisfaction degree and the life quality were compared between the two groups. Results The anal exhaust time, catheter removal time, first activity time and average hospitalization days in the observation group after the operation were significantly shorter than those in the control group; the average sleep time in the observation group was longer than that in the control group, P〈0.05. After the intervention, the scores of HAMA and HAMD in the two groups were significantly lower than that before the intervention. The scores of HAMA and HAMD in the observation group were significantly lower than those in the control group, P〈0.05. After intervention, the HPLP-Ⅱ scale of the two groups increased compared with that before intervention. The comparison between the two groups showed that the scores of HPLP-Ⅱ scale in the observation group were significantly higher than those in the control group, P〈0.05. Conclusion Whole systematic intervention can advance the efficacy of laparoscopic radical nephrectomy, relieve pains, aleviate depression or anxiety emotions. It is worthy of clinical promotion.
