摘要
目的探究分析开放式鼓室成形术与乳突根治术治疗慢性化脓性中耳炎的临床效果。方法选取2016年8月~2017年8月我院收治的116例慢性化脓性中耳炎患者作为研究对象,按照随机数字表法将其分为单纯组与联合组,每组各58例。单纯组患者采用单纯乳突根治术治疗,联合组患者采用开放式鼓室成形术与乳突根治术联合治疗。比较两组患者的治疗总有效率、术后外耳道形状变化以及干耳时间。结果联合组患者的治疗总有效率为98.28%,明显高于单纯组(86.21%),差异有统计学意义(P<0.05)。联合组患者的术后外耳道形状变化情况[正常率(93.10%)、小腔率(3.45%)、中腔率(3.45%)、大腔率(0.00%)]均优于单纯组[正常率(10.34%)、小腔率(34.48%)、中腔率(44.83%)、大腔率(10.34%)],差异有统计学意义(P<0.05)。联合组患者的术后平均干耳时间为(33.56±2.47)d,短于单纯组的(45.67±3.45)d,差异有统计学意义(P<0.05)。结论对于慢性化脓性中耳炎患者,采用开放式鼓室成形术与乳突根治术联合治疗的效果显著,可缩短术后干耳时间,提高患者的生活质量,值得在今后临床中进一步推荐使用。
Objective To explore the clinical effect of open tympanoplasty combined with mastoidectomy in the treatment of chronic suppurative otitis media. Methods A total of 116 patients with chronic suppurative otitis media who were admitted to our hospital from August 2016 to August 2017 were selected as the study subjects. According to the random number table method, they were divided into simple group and combined group, with 58 cases in each group.The simple group was given radical mastoidectomy alone, and the combined group was given open tympanoplasty combined with radical mastoidectomy. The total effective rate of treatment, the shape change of the external auditory canal after surgery, and ear dryness time were compared between two groups. Results The total effective rate in the combined group was 98.28%, which was significantly higher than that in the simple group（86.21%）, and the difference was statistically significant（P〈0.05）. Improvement of external auditory canal shape after surgery in the combined group（the normal rate [93.10%], the small cavity rate [3.45%], the mid-cavity rate [3.45%], the large cavity rate [0.00% ]） were superior to those in the simple group（the normal rate [10.34%], the small cavity rate [34.48%], the mid-cavity rate[44.83%], the large cavity rate [10.34%]）, and the differences were statistically significant（P〈0.05）. The average ear dryness time after treatment in the combined group was（33.56±2.47） d, which was significantly shorter than that in the simple group of（45.67±3.45） d, and the difference was statistically significant（P〈0.05）. Conclusion For patients with chronic suppurative otitis media, the effect of open tympanoplasty combined with mastoidectomy is significant. It can shorten the postoperative ear dryness time and improve the life quality of patients, which is worthy of further clinical application.
