肺复张联合早期俯卧位通气治疗重度急性呼吸窘迫综合征的效果观察 被引量:7

摘要 目的观察肺复张联合早期俯卧位通气治疗重度急性呼吸窘迫综合征(ARDS)的临床效果。方法将我院收治的93例重度ARDS患者按随机数字表法分为两组,对照组46例给予保护性机械通气治疗,观察组47例联合肺复张及早期俯卧位通气治疗,观察比较两组患者的治疗效果、呼吸动力学变化及预后情况。结果治疗后,观察组患者的氧合指数水平及Ⅰ级压疮发生率均高于对照组,PEEP、FiO_2水平、机械通气时间、ICU留置时间及28天病死率均低于对照组,差异均有统计学意义(P<0.05)。结论肺复张联合早期俯卧位通气能够提升重度ARDS患者氧合指数,降低病死率,改善预后效果。 Objective To observe the effect of the treatment of severe acute respiratory distress syndrome (ARDS) with pulmonary re extension combined with early prone position ventilation. Methods Randomly in our hospital 93 cases of patients with severe ARDS group,46 cases in control group were given protective mechanical ventilation treatment,the observation group of 47 cases with atelectasis and early prone ventilation treatment,the two groups were compared after treatment effect,prognosis and changes of respiratory dynamics. Results After treatment,the oxygen index and the incidence of pressure sores in the observation group were all higher than those in the control group.The levels of PEEP and FiO 2,mechanical ventilation time,ICU indwelling time and 28d mortality were all lower than those in the control group ( P 〈0.05). Conclusion Pulmonary extension combined with early prone position ventilation can improve the effect of oxygen improvement in severe ARDS patients,reduce the mortality,and be more beneficial to the prognosis.
