摘要
目的分析宫颈癌患者人乳头瘤病毒多重感染病原学特征,指导临床感染防治。方法收集243例宫颈癌患者临床资料,收集宫颈细胞液标本提取DNA并PCR扩增,检测HPV感染情况。采用HPV基因分型检测试剂盒对HPV分型,结果进行统计学分析。结果 243例宫颈癌患者HPV感染数为175例,感染率72.02%。其中,HPV单一感染患者148例,感染率为60.91%,HPV多重感染患者27例,感染率为11.11%。多重感染患者类型包括HPV 16+HPV 18感染患者11例,HPV 16+HPV 68感染患者4例,HPV 16+HPV 58感染患者2例,HPV 16+HPV 39感染患者2例,构成比分别为40.74%、14.81%、7.41%和7.41%;HPV 16+HPV 56、HPV 16+HPV 45、HPV 16+HPV53、HPV 16+HPV 31、HPV 58+HPV 43、HPV 58+HPV 68、HPV 16+HPV 18+HPV 39、HPV 16+HPV 45+HPV 68均为1例,构成比均为3.70%。不同年龄HPV多重感染患者,差异有统计学意义(χ2=6.9644,P=0.0083);不同民族HPV多重感染患者,差异无统计学意义(χ2=0.0329,P=0.8560);不同分期HPV多重感染患者,差异无统计学意义(χ2=0.1320,P=0.7164);不同病理分型HPV多重感染患者,差异无统计学意义(χ2=1.7312,P=0.1883);不同组织分化HPV多重感染患者,差异无统计学意义(χ2=0.1331,P=0.7152);不同淋巴结转移情况HPV多重感染患者,差异无统计学意义(χ2=0.1359,P=0.7123)。结论 HPV 16亚型是宫颈癌患者HPV多重感染中的优势类型,且HPV 16和其他亚群的多重感染在宫颈癌发生发展中存在协同作用。
Objective To analyze the etiological features of multiple human papillomavirus infection in patients with cervical cancer to guide the prevention and treatment of that infection in clinical settings. Methods Clinical data on 243 patients with cervical cancer were collected.DNA was extracted from cervical cytology specimens and was amplified with PCR to determine the status of HPV infection.HPV was typed using an HPV genotyping test kit,and results were statistically analyzed. Results Of 243 patients with cervical cancer,175 were infected with HPV for a rate of infection of72.02%.One hundred and forty-eight patients had a single HPV infection for a rate of infection of 60.91%,and 27 patients had a multiple HPV infection for a rate of infection of 11.11%.Multiple infections included HPV 16+HPV 18 in11 patients(40.74%),HPV 16+HPV 68 in 4(14.81%),HPV 16+HPV 58 in 2(7.41%),and HPV 16+HPV 39 in2(7.41%).One patient(3.70%)had HPV 16+HPV 56,1(3.70%)had HPV 16+HPV 45,1(3.70%)had HPV 16+HPV 53,1(3.70%)had HPV 16+HPV 31,1(3.70%)had HPV 58+ HPV 43,1(3.70%)had HPV 58+HPV68,1(3.70%)had HPV 16+HPV 18+HPV 39,and 1(3.70%)had HPV 16+HPV 45+HPV 68.Patients with a multiple HPV infection differed significantly in terms of age groups(χ2=6.9644,P=0.0083).Patients with multiple HPV infection did not differ significantly in terms of ethnic group(χ2=0.0329,P=0.8560),pathological stage(χ2=0.1320,P=0.7164),pathological classification(χ2=1.7312,P=0.1883),tissue differentiation(χ2=0.1331,P=0.7152),or lymphatic metastasis(χ2=0.1359,P=0.7123). Conclusion The HPV 16 subtype was the predominant type in patients with cervical cancer and multiple HPV infection,and multiple infection with HPV 16 and other subtypes had a synergistic effect on the development and progression of cervical cancer.
