摘要
目的分析及评价肠外营养(PN)处方,发现临床营养支持中的问题,旨在为不合理医嘱实施干预,提高临床营养支持合理性提供依据。方法采用回顾性分析方法,收集2015年1月至2017年11月PN处方共2 554张,将处方信息及配方组成录入自行设计的Excel表;并对组份合理性及配伍稳定性进行分析。结果糖脂比不适宜比例较高,占比为62.33%,热氮比适宜处方比仅为33.05%;一价阳离子合格率为100.00%;二价阳离子超标严重,尤其镁离子浓度合格率仅为49.37%;处方中胰岛素的应用比例达到85.43%。结论该院临床PN处方存在较大问题,应建立临床营养支持小组共同干预,同时强化药师的审方意识、提高审核技能,确保临床营养支持安全、合理、有效。
Objective To analysis and evaluation of parenteral nutrition （PN） prescription, the problems in clinicalnutrition support were found, which was aimed to provide basis for the intervention of irrational medical orders and improvingthe rationality of clinical nutrition support. Methods Retrospective analysis method was used,2 554 PN prescriptions fromJanuary 2015 to November 2017 were coueced, and the prescription information and formula composition were recorded inthe self-designed Excel sheet. The rationality of the components and the compatibility stability were analyzed. Results Theinaptitude ratio of glycolipid ratio was higher, accounting for 62.33%; and the ratio of thermal nitrogen ratio was 33.05%. Thepass rate of one valence cation was 100.00%; the divalent cation exceed the standard seriously, especially the qualified rateof magnesium ion concentration was only 49.37%. The proportion of insulin used in prescription was 85.43%. ConclusionThe clinical PN prescriptions in this hospital have some important problems, and a clinical nutrition support group should beestablished to intervene it. At the same time, it is necessary to strengthen the awareness of the pharmacists and improve theexamination skills to ensure that the clinical nutrition support is safe, reasonable and effective.
