摘要
目的探讨植入式心电事件监测器(ICM Reveal LINQTM)在晕厥门诊中对心律失常引起晕厥的诊断价值。方法选择2017年1月1日-2018年5月1日在新疆医科大学第一附属医院晕厥门诊就诊的心源性晕厥待查患者46例,其中植入ICM的患者15例,植入后通过自动记录和事件激活两种方式规律随访,分析患者再次发生晕厥、检出心律失常等情况。结果植入ICM者较未植入ICM者就诊次数减少,检查次数减少,平均检查花费降低(P<0.05)。植入ICM者随访期间共检出心律失常12例:缓慢型心律失常5例,其中心脏停搏3例;快速型心律失常7例,其中房颤伴长间歇1例。结论 ICM对不明原因晕厥患者病因诊断和制定进一步治疗方案提供了可靠的依据。
Objective To investigate the diagnostic value of insertable cardiac monitor（ICM Reveal LINQTM）for syncope in the syncope clinic.Methods 46 patients with cardiac syncope who were examined in the syncope clinic of the First Affiliated Hospital of Xinjiang Medical University from January 1,2017 to May 1,2018 were consecutively enrolled.Among them,15 patients with implanted ICM were followed up regularly by auto-recording and event activation after implantation,and their syncope and arrhythmia date were analyzed.Results Patients implanted with ICM had fewer visit number to hospital,fewer examinations,and lower average costs than those without implantment（P 0.05）.There were 12 cases of arrhythmias,5 cases of slow arrhythmias（including 3 cases of cardiac arrest）,and 7 cases of rapid arrhythmia（including one case of atrial fibrillation with long interval）were detected in the follow-up period of ICM patients.Conclusion ICM provides a reliable basis for etiological diagnosis of patients with unexplained syncope and the development of further treatment options.
