摘要
目的系统评价高压氧(hyperbaric oxygen,HBO)治疗突发性聋伴耳鸣的疗效。方法计算机检索CNKI、万方、维普、CBM、Pub Med、EBSCO、OVID等中英文数据库,全面收集高压氧治疗突发性聋伴随耳鸣的随机对照试验(RCT),检索时限均为建库至2017年1月。由两位研究者共同评价所纳入研究的质量,并采用Rev Man 5.2软件进行Meta分析。结果共纳入9个试验533例患者,均为非安慰剂随机对照试验,所有试验均是根据HBO治疗后耳鸣症状缓解的有效率评估疗效,采取二分类变量方法进行Meta分析,结果提示:HBO组耳鸣症状改善明显优于对照组[RR=1.63,95%CI(1.47,1.80)];剔除其中一个权重大的试验,其余8个试验作Meta分析,仍提示HBO组耳鸣症状改善明显优于对照组[RR=1.65,95%CI(1.46,1.86)];所有研究均未报道不良事件的发生。结论基于当前证据,高压氧对突发性聋伴随耳鸣有一定疗效,但由于纳入研究数量少、质量低,本研究结果的证据强度势必受影响,故有必要进一步开展高质量、大样本随机对照试验来进一步评价其疗效。
Objective To systemically assess the efficacy of hyperbaric oxygen treatment （ HBO） for patients with sudden deafness and tinnitus.Methods Randomized controlled trials （ RCTs） about hyperbaric oxygen treatment with placebo or open control for patients with sudden deafness and tinnitus were comprehensively retrieved in data-base such as PubMed,OVID,EBSCO,wanfang, CBM,CNKI and YIP.The retrieval was started from data-base set-up until to January 2017.Two reviewers independendy screened the quality of literatures.Then, meta-analysis was performed using RevMan 5.2.Results A total of 9 randomized non-placebo controlled trials involving 533 patients were included.The effect evaluation was performed according to tinnitus symptom relief after HBO treatment and then the Meta analysis was performed by using binary variable method.The results showed that the improvement of tinnitus in the HBO group was better than that in the control group （ RR= 1.63,95% CI= 1.47,1.80 ）.The results also showed that the improvement of tinnitus in the HBO group was still better than that in the control group after removing a weighty trial （ RR= 1.65,95% Cl 1.46,1.86） .However,no severe adverse events occurred.Conclusion Current evidence indicates that,HBO can effectively reduce the score of sudden deafness with tinnitus.It5s necessary to further carry out high quality randomized controlled trials with large sample size to as-sess its effectiveness.
