The Effect of Disease Severity on Intestinal Barrier Function in Patients with Aneurysmal Subarachnoid Hemorrhage

摘要 目的探讨动脉瘤性蛛网膜下腔出血(aneurysmal subarachnoid hemorrhage,a SAH)行介入治疗患者不同病情程度对肠道屏障功能的影响。方法选择我院2015年1月—12月收治的35例a SAH,按Hunt-hess分级标准分为轻度组(Hunt-hess分级1~2级)和中重度组(Hunt-hess分级>2级),分别在术前及术后1、3、7、14 d检测血二胺氧化酶(DAO)、D-乳酸(DLC)的水平变化。结果所有患者术前及术后DAO水平均高于正常值(DAO正常参考值≤10μ/L),术后7 d达到高峰,术后14 d水平下降;术前DLC水平则均在正常范围内(DLC正常参考值≤15 mg/L),术后1 d开始升高且高于正常值,术后7 d达到高峰,术后14 d下降。与本组术前比较,术后1、3、7 d的DAO、DLC及术后14 d的DLC水平升高,比较差异有统计学意义(P<0.05);两组术后3 d的DLC水平比较差异有统计学意义(P<0.05)。结论 DAO与DLC水平与a SAH患者行介入治疗后的病情程度密切相关,病情越重肠道屏障功能损伤越大。 Objective To investigate the effect of disease severity on intestinal barrier function in patients with aneurysmal subarachnoid hemorrhage (aSAH) after interventional therapy. Methods A total of 35 patients with aSAH who were treated in our hospital from January 2015 to December 2015 were divided into mild group (Hunt-hess grade I^II) and moderate-to-severe group (Hunt-hess grade〉II), according to Hunt-hess grading system. The level of diamine oxidase (DAO), D-lactic acid (DLC) were measured before surgery, and at 3, 7 and 14 d after surgery. Results The DAO level of all patients was higher than the normal value before and after surgery (the normal reference value of DAO was ≤10μ/L ), and reached the peak at 7 d after surgery, and decreased at 14 d after surgery. Before surgery, DLC level was within the normal range (the normal reference value of DLC was ≤ 15 mg/L), which began to increase at 1d postoperatively and was higher than normal, peaked at 7 d postoperatively, and decreased at 14 d postoperatively. Compared with those before surgery, the levels of DAO, and DLC at 1, 3, 7 d postoperatively and the level of DLC at 14 d postoperatively were significantly increased ( P 〈0.05). However, there was statistically significant difference in the level of DLC at 3 d after surgery between the two groups ( P 〈 0.05). Conclusion DAO and DLC levels are closely related to the disease severity of the patients with aSAH after interventional therapy. The more severe the disease, the greater the impairment of intestinal barrier function.
