摘要
目的观察颈部肿瘤体积缩小及消瘦等因素导致的颈部组织结构的空间位移及放射剂量曲线覆盖靶区的变化。方法选取2017年1月~2018年3月期间我院收治的头颈部肿瘤分段调强放疗的50例患者,所有患者均需照射颈部肿瘤。研究患者首次CT定位图像及再次定位CT图像中颈部组织结构(包括肿瘤病灶)的空间位移及剂量曲线覆盖靶区的变化。结果第二阶段放疗计划的目标靶区界线相比与第一阶段放疗计划,在三维空间X、Y、Z轴位方向上移动度各不相同,X轴及Y轴方向上差异较为显著(P<0.05),而Z轴方向上差异无统计学意义(P>0.05)。结论实施分段调强放疗技术在治疗颈部肿瘤放疗中具有重要价值,为优化颈部肿瘤病灶的调强放疗技术提供依据,为临床医生对颈部肿瘤治疗时靶区的勾画提供具有更现实意义的参考依据。
Objective To observe the spatial displacement of the cervical tissue structure and the change of the radiation dose curve covering the target area caused by shrinkage and wasting of neck tumor.Methods A total of 50 patients with head-neck tumors treated who were treated with segmental intensity-modulated radiation therapy in our hospital from January 2017 to March 2018 were selected.All patients required irradiation of cervical tumors.The spatial displacement and the change of dose curve of the cervical tissue structure(including tumor lesions) in the first CT-localized image and re-localized CT image covered the target area.Results Compared with the first-stage radiotherapy plan,the target range boundary of the second-stage radiotherapy plan was different in the three-dimensional space X,Y,and Z axis directions,and the difference between the X-axis and Y-axis directions was significant(P〈0.05),but there was no significant difference in the Z axis direction(P〉0.05).Conclusion The segmented intensity-modulated radiation therapy is of great value in the treatment of cervical tumor radiotherapy,which provides a basis for optimizing intensity-modulated radiotherapy for cervical tumor lesions and provides clinicians with a more realistic reference basis for the drawing the target area during neck tumor treatment.
