摘要
目的探讨不同剂量维生素D预防早产儿佝偻病的效果及相关生化指标的变化。方法回顾性分析2016年6月至2017年6月70例早产儿的临床资料,按照随机数表法将其分为两组,对照组早产患儿给予低剂量维生素D治疗,观察组早产患儿给予高剂量维生素D治疗。比较两组治疗效果、骨钙素、骨源性碱性磷酸酶及25-羟基维生素D、血钙水平。结果治疗后观察组早产儿佝偻病患病率为8.57%,低于对照组的22.86%,差异有统计学意义(P<0.05);治疗后两组早产儿的骨钙素、25-羟基维生素D、血钙的水平均高于治疗前,骨源性碱性磷酸酶低于治疗前,观察组早产儿的骨钙素、骨源性碱性磷酸酶水平低于对照组,20-(OH)D及血钙水平高于对照组,差异有统计学意义(P<0.05)。结论高剂量维生素D可明显降低早产儿佝偻病发生率,改善骨钙素、骨源性碱性磷酸酶及25-羟基维生素D的水平,提高血钙水平,值得临床推广与应用。
Objective To investigate the effects and related biochemical indexes of different doses of vitamin D on preventing premature rickets. Methods The clinical data of 70 cases of premature infants from June 2016 to June 2017 were retrospectively analyzed. They were divided into two groups according to the random number table method. The infants in the control group were treated with low dose of vitamin D,and the infants in the observation group were treated with high dose of vitamin D. The therapeutic effects,the levels of osteocalcin,bone alkaline phosphatase,25-( OH) D and blood calcium were compared between the two groups after treatment. Results After treatment,the incidence of rickets in the observation group( 8. 57%) was lower than that in the control group( 22. 86%),the difference was significant( P〈0. 05); after treatment,the levels of osteocalcin,25-( OH) D and blood calcium in the observation group were higher than those before the treatment,the levels of bone alkaline were lower than those before treatment,the levels of osteocalcin and bone alkaline in the observation group were lower than those of the control group,the levels of 25-( OH) D and blood calcium were higher than those of the control group,,the differences were significant( P〈0. 05). Conclusions High dose of vitamin D can effectively reduce the prevalence of infantile rickets, improve the levels of osteocalcin, bone alkaline phosphatase and25-( OH) D,and enhance the levels of serum calcium,which is worth of clinical promotion.
