摘要
目的探讨盐酸右美托咪定联合瑞芬太尼在骨科手术中的应用效果。方法选择东莞市麻涌人民医院2015年7月—2017年12月收治的120例行区域阻滞的骨科手术患者。按照随机数字表法分为观察组和对照组,每组各60例。对照组给予丙泊酚复合瑞芬太尼辅助麻醉,观察组给予盐酸右美托咪定复合瑞芬太尼辅助麻醉,观察两组麻醉效果。结果对照组脉搏氧饱和度在T2和T3时间段明显低于观察组,且呼吸频率在T1、T2和T3时间段较观察组明显下降,组间比较,差异有显著性;两组心率、平均动脉压指标在各时间段差异无统计学意义;观察组不良反应发生率(11.7%)明显低于对照组(25.0%),组间比较,差异有统计学意义;两组患者各时间点OAA/S评级情况对比,差异无统计学意义。结论盐酸右美托咪定联合瑞芬太尼带来较满意的麻醉效果的同时,可有效降低术中不良反应的发生率,促进手术顺利完成,患者更加舒适。
Objective To explore the effect of dexmedetomidine combined with remifentanil in operation in Department of Ortho- pedics. Methods 120 patients who underwent regional block operation in Department of Orthopedics from July, 2015 to December, 2017 in the hospital were selected. According to the random number table method, they were divided into observation group and con- trol group, with 60 cases in each group. The control group was given propofol combined with remifentanil assisted anesthesia, and the observation group was given dexmeimidin combined with remifentanil assisted anesthesia. The effect of two groups was observed. Re- suits The pulse oxygen saturation in the control group was significantly lower in T2 and T3 periods than in the observation group, and the respiratory frequency was significantly lower in T1, T2 and T3 than in the observation group. The difference was significant be- tween the groups. The heart rate and mean arterial pressure in the two groups were not significantly different in each time period. The adverse reaction in the observation group （ 11.7% ） was significantly lower than that of the control group （25.0%）, and the difference between groups was significant. The difference of OAA/S rating at each time point of the two groups was not significant. Conclusion Dexmeimidine hydrochloride combined with remifentanil has a satisfactory anesthesia effect, which can effectively reduce the inci- dence of adverse reactions in the operation, promote the successful completion of the operation, and patients more comfortable.
