摘要
[目的]总结宣桂琪名老中医诊治小儿癫痫的学术思想及临床经验。[方法]跟随宣老抄方侍诊,收集相关医案,总结宣老运用中医疗法治疗小儿癫痫的临床诊疗思路及用药经验,并以典型医案加以分析。[结果]宣桂琪名老中医认为小儿癫痫的病因不外乎"风""火""痰""瘀""虚",临床上病机复杂多变,包括风火攻上、痰瘀互结、虚实夹杂等,治疗宜分清本末虚实。风邪为主则祛风平肝,火邪偏盛则清热降火,痰瘀互结则化痰祛瘀,虚象明显则补益脾肾。宣老自拟"宣氏癫痫方",将五种病因及复杂病机均考虑在内,处方配伍严谨,临床疗效显著。所举医案,具体展示了上述辨证思路和治疗经验。[结论]宣桂琪名老中医对于小儿癫痫认识深刻,自拟"宣氏癫痫方"疗效显著,对临床有指导意义,值得学习借鉴。
[Objective]To summarize the famous TCM doctor XUAN Guiqi＇s academic thoughts and clinical experience in diagnosing and treating epilepsy of children.[Method]Through copying prescriptions, collecting relative cases, summarizing clinical thoughts and experience of herbs use. A typical case is adopted to support. [Result]Famous TCM doctor XUAN Guiqi deems that the causes of children ＇s epilepsy are no more than ‘wind＇‘fire＇‘phlegm＇‘stagnation＇‘deficiency＇. The pathomechanism is complicated in the clinic, including‘wind and fire attacking upwards＇ ‘intermingled phlegm and stasis＇ ‘deficiency-excess complex＇and so on. Thus‘root＇‘tip＇‘deficiency＇‘excess＇ should be distinguished and herbs should be varied according to different syndromes in the treatment. For example, facing the ‘wind＇evil, we should use the therapy of dispelling wind and calming liver. Clearing heat and reducing fire aiming a‘theat＇evil, dissolving the phlegm and dispelling the stasis aiming a‘tintermingled phlegm and stasis＇, tonifying spleen and kidney aiming at‘deficiency＇syndrome. Self-made XUAN Shi Epilepsy Formula takes five causes and complicated pathomechanism into consideration, in which the combination is rigorous and the clinical efficacy is obvious. [Conclusion]Famous TCM doctor XUAN Guiqi has a deep understanding of children ＇s epilepsy, XUAN Shi Epilepsy Formula has obvious curative effect, which has guiding significance in the clinic. It is worthwhile to be learned and used for reference.
