摘要
目的观察脑电双频指数(BIS)监测应用于老年腹腔镜手术患者的临床价值。方法选择老年腹腔镜手术患者80例,按随机数字表法分为观察组和对照组,每组40例。观察组采用BIS监测术中麻醉深度,对照组根据临床经验实施麻醉,观察两组丙泊酚用药量、苏醒时间及拔管时间,并采用简易精神状态量表(MMSE)对患者术前1天(d0)、术后1天(d1)、术后3天(d2)、术后7天(d3)进行评分。结果观察组丙泊酚用量少于对照组(P <0. 05);观察组苏醒时间及拔管时间比对照组短(P <0. 05);与d0时比较,对照组及观察组d1、d2时MMSE评分降低,对照组下降更明显(P <0. 05);观察组术后认知功能障碍发生率(30. 00%)低于对照组(55. 00%),P <0. 05。结论脑电双频指数监测用于老年腹腔镜手术患者,能减少丙泊酚的用量,缩短苏醒时间及拔管时间,降低术后认知功能障碍发生率。
Objective To observe the clinical value of bispeetral index （BIS） monitoring in elderly patients undergoing laparoseopie surgery-. Methods Eighty- elderly patients with laparoseopie surgery- were selected. These eases were randomly divided into the observation group and the control group（ n =40 each）. BIS was used to monitor the depth of anesthesia in the observation group. The control group was anesthetized according to clinical experience. Dosage of propo- fol ,wake time and extubation time were observed in the two groups ,and Mini-Mental State Examination （MMSE） was used to evaluate the result at 1 day （ do ） before operation and 1 day （ d2 ） ,3 day （ d2 ） ,7 day （ d3 ） after operation. Results The amount of propofol in the observation group was lower than that in the control group （ P 〈 0.05）. The wake time and extubation time of the observation group were shorter than those of the control group （P 〈 0.05 ）. Compared with do , The score of MMSE decreased in both two groups at d1 and d2, which decreased more significantly in the control group （P 〈 0.05）. The incidence of POCD in the observation group （30.00%） was lower than that in the control group （55.00%） （ P 〈 0.05） after operation. Conclusion Bispeetral index monitoring in elderly patients undergoing laparoseopie surgery- can reduce the dosage of propofol, shorten the wake time and extubation time ,and improve postoperative cognitive.
