摘要
目的了解苏州工业园区某社区2型糖尿病患者规范化管理的效果,探讨对2型糖尿病患者进行社区健康干预的有效途径。方法选择苏州工业园区某社区2014—2015年登记管理的2型糖尿病患者200例,随机纳入规范化管理组和常规管理组,各100例。1年以后评价两组患者管理前后血糖及糖化血红蛋白(Hb A1c)水平、糖尿病相关知识的掌握和自我管理能力的变化情况。结果规范化管理组患者的各项疾病相关指标均比接受管理前有明显改善(P<0.05),也比同期的常规管理组患者好(P<0.05);规范化管理组患者糖尿病相关疾病的知识掌握情况和自我管理能力评价也高于对照组(P<0.05)。结论规范的社区健康干预,能够有效提高患者对疾病相关知识的掌握和自我管理能力,进而有效提高患者的血糖达标率。
Objective To understand the effect of standardized management of patients with type 2 diabetes in a community of Suzhou Industrial Park, and to explore an effective approach to community health intervention for type 2 diabetes patients. Methods A total of 200 patients with type 2 diabetes in a community of Suzhou Industrial Park from 2014 to 2015 were randomly divided into the standardization management group（100 cases） and the routine management group. One years later, the changes of blood glucose and glycosylated hemoglobin（Hb A1 c） level, knowledge of diabetes, and self-management ability of two groups of patients before and after management were evaluated. Results The disease related indicators of patients in standardized management group were significantly improved compared with those before receiving management（P〈0.05）, which was also better than those in conventional management group（P〈0.05）; The levels of diabetes related diseases knowledge mastery and self-management ability of patients in the standardized management group were also higher than those in the control group（P〈0.05）. ConclusionStandardized community health intervention can effectively improve the patient’s knowledge of disease related and self-management ability, and thus effectively improve the patient’s blood sugar compliance rate.
