摘要
目的分析郑州地区不同遗传体征育龄期女性血清叶酸、维生素B12及同型半胱氨酸(HCY)的水平,为郑州地区育龄女性补充叶酸提供参考。方法选取2015年5月至2017年7月于郑州大学第三附属医院就诊的1 190例健康育龄女性为研究对象。采集研究对象口腔黏膜上皮细胞,用柱式抽提盒提取样本DNA,采用Taqman-MGB探针对5,10-亚甲基四氢叶酸还原酶(MTHFR)C677T、A1298C和甲硫氨酸合成酶还原酶(MTRR)A66G 3个位点分别进行荧光定量PCR检测。根据3个位点的多态性分为高度风险组、中度风险组、低度风险组、未发现风险组,采用直接化学发光法及循环酶法测定研究对象外周血血清叶酸、维生素B12及HCY水平。结果育龄期女性血清叶酸、维生素B12及HCY水平分别为(15.65±6.56)μg/L、(411.85±155.16)ng/L、(7.91±3.16)μmol/L。高度风险组及中度风险组血清叶酸水平分别低于低度风险组与未发现风险组,差异有统计学意义(P<0.05)。高度风险组血清HCY水平高于未发现风险组,差异有统计学意义(P<0.05),4组血清维生素B12水平差异无统计学意义(P>0.05)。结论不同风险组血清叶酸、维生素B12及HCY水平存在差异,需根据不同的遗传体征针对性增补叶酸;郑州地区女性叶酸利用能力较差,高度及中度风险人群占比较高。
Objective To analyze the levels of serum folic acid,vitamin B12 and homocysteine in women with different genetic signs in the region,and provide reference for folic acid supplementation in women of childbearing age. Methods One thousand one hundred and ninety healthy women of childbearing age were selected from May of 2015 to July of 2017 in the Third affiliated Hospital of Zhengzhou University. The oral mucosal epithelial cells were collected and the sample DNA was extracted using a column-type extraction cartridge. The Taqman-MGB probe was used to probe PCR of 5,10-methylenetetrahydrofolate reductase( MTHFR) C677 T,A1298 C,methionine synthase and three sites of reductase( MTRR) A66 G. According to the polymorphisms of three loci,patients were divided into high-risk group,moderate-risk group,low-risk group,and no-risk group. The levels of serum folic acid,vitamin B12 and HCY were measured by direct chemiluminescence and cyclodextrin method. Results The levels of serum folic acid, vitamin B12 and homocysteine were( 15. 65 ± 6. 56) μg/L、( 411. 85 ±155. 16) ng/L、( 7. 91 ± 3. 16) μmol/L,respectively. The levels of serum folic acid in high risk group and moderate risk group were lower than those in low risk group and no risk group,and the difference was statistically significant( P〈0. 05). The serum homocysteine level in high risk group was higher than that in no risk group,and the difference was statistically significant( P〈0. 05). There was no significant difference in serum vitamin B12 level in all risk groups( P 〉0. 05). Conclusion There are differences in serum folic acid,vitamin B12 and HCY levels in different risk groups. It is necessary to add folic acid for women of childbearing age according to different genetic signs.
